s we move into the season, the clean, crisp air makes for a refreshing transition from the lethargic humidity—left by the final days of summer—to the dry and invigorating breaths that come with autumn hikes and field-shopping during the last days of the harvest. October, with its glowing environment accessory of colorful foliage, accented by a backdrop of evergreens, creates a landscape that resembles nature’s last smile of the year. The natural beauty offered by these colorful canopies serves as a comforting layer of security placed upon the earth, which has grown tired from a challenging year and must now rest until it is time to wake refreshed in the spring. As exhilarating as the weather can be, it tugs at our desire to become wrapped in a heavy sweater or warm fleece and enjoy an outdoor crackling fire with friends and family. Toasting with piping hot mulled cider or wine—with a cinnamon stick, tasting a delicious slice of pumpkin pie—or better—enjoying a feast of fresh game and fall vegetables served in rustic fashion; these are the events that create memories and traditions. Many will be moving into new homes; countless others will be shopping for another residence, while a significant number will settle into a neoteric lifestyle, compatible with telecommuting. With migration comes the enjoyable task of designing a new space, and because homes have become centralized— for both social and work activities; creativeness has no bounds. Moving on to advances in the treatment of COVID-19, we report on how Brigham and Women’s Hospital of Boston is in the middle of testing a vaccine for the disease. It is exciting news, but with it comes controversy. It appears that healthcare administrators involved with medically related companies have raised their of the media. The Boston Globe has scrutinized some highly regarded CEOs for their involvement with these firms, leading to possible conflicts of interest with the biotech industry. Under pressure, one leader was recently forced into resigning from her board position. The question for debate is, how close is too close? Violence seems to be out of control, but who is responsible? Writer Rob Saint Laurent explores the science and reports on the underdeveloped brains of young adults. He looks at the research to determine if individuals breaking things should be responsible for their actions, or are they ill-equipped to make life and death decisions? he Paquachuck Inn has an illuminating history and is available for someone interested in carrying on its prominence in connecting the past, present, and future. The word Paquachuck is translated to quahog and is steeped in Native American culture. Historical documents show that members of the Wampanoag tribe settled along the fertile banks and sparkling rivers of Westport due to suppression by colonists in Dartmouth. The displaced people made their way to Horseneck—a State Reservation and incredible beach; Coaksett, “Land on the other side of the little land,” now referred to as Acoaxet, and Noquochoke, “Land at the fork of the river.” Finding a new home with an abundance of food, water, shelter, and the natural beauty of the surrounding area, the new inhabitants must have been quite satisfied—as is today’s homeowners. While much has changed, what has not, is the demand for the limited inventory of historically significant homes and property in a town that draws visitors and buyers worldwide. The building, found at the very end of Main Road on Westport Point, is tied to the whaling industry, responsible for creating commerce and vast wealth for its residents, ship owners, and others who sought adventure on the high seas. Originally constructed in 1820, the multi-story building served as a sail loft on the second floor and chandler on the ground level. Close to the water, both sailors and vendors found added convenience to conduct trade; making the location a valuable investment. Today, the “Point” is home to year-round and summer residents. It is a quiet area of well-to-do citizens, professional artists, and a local fishing fleet. At the tip of Westport, activity abounds with pleasure and commercial watercraft of all descriptions. Within the snug harbor and inlets, small piers dot the area and allow easy access for launching kayaks, canoes, and a local favorite—Beetle Cats. As the whaling industry waned, so did the need for equipping ships; and the building went quiet during the first half of the 20th century. Finally, around 1950, life sprang back into the Paquachuck, and it became useful once again as both a private home and an Inn. The property is listed on the National Registry of Historic Places and deserves acknowledgment, considering the original building has been preserved since it was built. Even with wild speculation about Tesla’s viability and long-term success, its stock price (TSLA ↑442.15 / 9-18-20) tells a different story and seems to indicate people are investing in the person— not the car. Similarly, the accuracy of the CDC tracking protocol used for accounting the spread of the Coronavirus pandemic is pinned to notions of questionable reliability and accuracy. It, too, has created doubt about the disease’s etiology, spread, and the efficacy of treatments. With vaccines in the final stages of approval and an expectation of distribution within months—if not weeks—biomedical companies appear poised to make good on their promise.

The public has learned to question the integrity of the media, social networks, and political narratives; for, with all projections and predictions, an idiom of salt should be served. Talk remains cheap, while actions ring the register and improve our lives. It will be interesting to evaluate my original thesis a year from now. Tesla deserves credit for its accomplishments thus far; they exemplify the human race’s ability to think beyond today and project what needs to be learned tomorrow. With an eye on the health and welfare of future generations, Tesla and other industries —lead by Musk—are at the forefront of the next industrial, technological, and explorational revolution. Musk, with his ingenuity, unorthodox ways, unconventional approach to life and work, and his quest to achieve a worldwide ‘zero-emissions future,’ has earned the respect of millions. But, his insight into advanced neurotechnology, Artificial Intelligence (AI),

space-age public transportation, and the colonization of Mars make Musk a person worth watching. Tesla, perched high above the pedestrian notion that Electric Vehicles (EV) must be boring, ugly, and continuously plugged into an outlet, has broken the mold and hit a triple by offering—or soon to be—superior energy storage, unfathomable efficiency, and state-ofthe-art styling. TOO CLOSE FOR COMFORT: WHO SHOULD DECIDE? BY STEVEN CHAN

F

rom the sounds emanating from the federal government, market analysis, and the White House, you might believe that the public will have the first round of vaccines coming out somewhere between October and the end of the year. The scramble is on, with one political party driven to get the preventative treatment to pharmacies before election day; the other, throwing shade on its effectiveness without knowledge of

the final test results. What matters is that Boston’s health care community is working in the trenches to deliver an effective vaccine as soon as possible—albeit with a caveat. In late July, Brigham and Women’s Hospital (BWH) announced their leadership roll in the testing of COVID-19 vaccine trials. Through communication sent to media sources, the hospital indicated they advanced their effort to conduct a 30,000 subject phase 3 trial, including a phase 3 COVE study evaluating mRNA-1273. 50 | OCTOBER 2020 | NEW ENGLAND NEWS + MEDIA

The vaccine, manufactured by Moderna, located in Cambridge— and now practically a household name, may become the shining star in the battle against COVID-19. The study will include participants from around the country, with the hospital acting as a clinical partner, in conjunction with the COVID-19 Prevention Network (CoVPN). From a press release, we learned that Lindsey Baden, MD, an infectious disease specialist and vaccine expert, will serve as a co-principal in the study. “This is the first phase 3 trial that will test the effectiveness of the vaccine against COVID-19, and we have been working diligently across [the] industry, academia, and government to pave a path for launching this trial rapidly and safely,” said Baden. “Our goal has been to not only launch this important trial but also to develop a rapid, high-quality process for vaccine trials that are to come.” The COVE study is under the umbrella of Operation Warp Speed (OWS), having a goal of delivering 300 million doses of a tested and proven vaccine for COVID-19 as early as 2021. OWS is part of a partnership under the Department of Health and Human Services, the Centers for Disease Control and Prevention, the FDA, the NIH, BARDA, and the Department of Defense.

The purpose of continuing to monitor participants is to look at long-term immune responses to SARS-CoV-2. With all this good news, it has not been easy sailing for the hospital and its leader. Officials recently disclosed that the president of BWH, Dr. Betsy Nabel, was a member of Moderna’s Therapeutics Board of Directors since 2015 and had a financial interest in the drug company. In a statement issued by the hospital, they explained the relationship between Nobel and Moderna was reviewed, and satisfactorily meets all standards, policies, and procedures following the Mass General Brigham conflict of interest guidelines. BWH went further to shine light between Nabel, the hospital, and the drug company. They pointed out that the CEO was not—at any time—acting as an agent for the hospital and her function at Moderna was personal. Additionally, the healthcare organization had a plan in place, outlining how Nabel would recuse herself from any matter relating to the joint study. Further distancing included the disclosure that BWH is “to be one of 89 sites involved,” and that “all…key aspects of the trial,” as well as, “Dr. Nabel’s relationship with Moderna was fully disclosed to the National Institutes of Health.” It should be noted that Nabel is compensated for her service on the Moderna Board. With all of this, the pressure continued to mount. According to BWH, articles published by The Boston Globe were said to be “inaccurate” and “misleading,” as to Nabel’s relationships with the pharmaceutical industry and research conducted by hospitals. The newspaper addressed others in the upper echelons of medicine and pointed out other hospital administrators with questionable affiliations with high profile research conglomerates. The series focused on the inappropriateness of the many cozy relationships between CEOs of medical non-profits and profit earning drug and biotech companies. According to Erin McDonough, senior vice president, and chief communications officer for BWH, the hospital’s board was satisfied with safeguards set up to monitor and evaluate the ongoing relationship between the hospital’s president and Moderna. Yet, the damage had been done, with public opinion turning negative. The view that there was a conflict of interest was hefty enough to cause Nabel to resign from Moderna’s board on July 30, 2020.

In the final days, Dr. Nabel issued a statement explaining her position and reason for stepping down from the company in question. “As a physician and scientist, I have dedicated my life to the health and well-being of others. Despite the management plans that have been put in place to ensure the COVID-19 vaccine study is not compromised due to my connection with Moderna, I have come to realize that those who do not know me, or how such trials are conducted, may perceive a conflict of interest. It is critically important that the pubic trust the conduct and outcome of the vaccine trials, so in the best interest of the greater good, I have made the difficult decision to resign from the Moderna Board.” As we wait to see the results of this study, the debate will continue over public and private interactions. During this pause, all involved could benefit from a conversation focusing on who’s responsible for navigating these troubled waters; the media, or should these concerns be handled from within healthcare organizations? Quite noticeably, even when the rules of conduct are spelled out and supervised, conflict over personal rights and responsibility remains a hot issue in Bean Town. Perhaps, we need to ask ourselves whether hospital leaders should be prevented from serving on boards because a conflict of interest might result, or consider whether a claim of wrongdoing is being manufactured outside the laboratories and boardrooms for other reasons. C

rime statistics among US juveniles and young adults were a mixed bag as of 2018. According to the US Department of Justice, overall arrests of juveniles, age 17 and younger, have dropped roughly 70 percent from a peak of almost 2.7 million in 1996.1 However, murder arrests of older juveniles, ages 15-17, have risen since 2012, with a 21 percent increase between 2014 and 2018. At the same time, motor vehicle theft, fraud, and embezzlement arrests in this age group have also risen significantly.1 Juvenile arrest rates have declined for both males and females over the past ten years, while females have seen an overall increase since 1980. And while white and Native American youth are more prone to liquor law violations, black older youth are much more prone to violent crime.2 Since 1996, arrests of adults have declined roughly 21 percent. Yet among young adults, ages 18-20 and 21-24, violent crime arrests rose at a higher rate than older juveniles.1-2 With these trends at risk of being affected by today’s intensifying civil unrest—what might cause young people to exhibit bad judgment and commit crimes—especially violent acts motivated by strong beliefs? THE IMMATURE BRAIN

A reason may lie behind the eyes and between the ears in how young people process what they see and hear. Though the law says age 18 marks adulthood, nature seems to say otherwise. Decades of research suggest it can take 25-30 years (some even say 40) for the human brain to fully mature, depending on gender and individual traits. As reported by BBC News Services, scientists at last year’s London meeting of the Academy of Medical Sciences explained that the transition from adolescence to adulthood is a much more gradual, finer process than what we’ve believed to now. This slow process can influence young people’s behavior and increase their risk of mental health disorders.3 Said Professor Peter Jones of Cambridge University, “To have a definition of when you move from childhood to adulthood

looks increasingly absurd,” yet we say an 18-year-old can vote, buy alcohol, and be treated as an adult when arrested probably out of institutional convenience, he explained.3 In a comprehensive literature review in the journal Neuropsychiatric Disease and Treatment, the authors note that multiple MRI studies have shown that the development of nerve-protective myelin sheaths needed for proper brain function continues through adolescence, ages 10-24.4 Two of the brain’s important structures, the limbic system and prefrontal cortex of the frontal lobe, both remain immature through adolescence. The limbic system is thought to involve self-control, emotions, and decision making. Simultaneously, the closely related prefrontal cortex is believed to be involved in many complex behavioral processes and personality development. Both systems are impacted by hormones; heredity; environment; pharmacology (alcohol, caffeine, etc.); physical, economic, and psychological stress; and other factors.4 An immature limbic system and prefrontal cortex underlie youth tendency for excitability, impulsive behavior, bad judgment, and social maladjustment, the authors explain.4 “It is well-established that the brain undergoes a ‘rewiring’ process that is not complete until approximately 25 years of age.”4 THE NEUROSCIENCE OF ADOLESCENT CRIME

Renowned youth brain development expert Lawrence Steinberg, Ph.D., is in alignment with Peter Jones when it comes to separating adolescent criminal behavior from that of adults. Laura H. Carnell, Professor of Psychology at Temple University, and Steinberg contend that because adolescent brains are still developing, they may have less control over their behavior than is currently believed. He believes that while adolescents should be punished accordingly, they should be re-examined as adults.5 His research indicates the following about developing adolescent brains: A LACK OF IMPULSE CONTROL. Mind the Gap BY ANDREW AARON, LICSW

a spark plug; at the tip is a gap between the terminal and a small arm that reaches above. For the part to function properly, a spark must jump between the two posts and create a connection, causing the engine to start. If debris lodges between the terminal and the arm, the spark will not fire as required, leaving the engine to lay idle. This anecdote is an appropriate metaphor for a love relationship; where there also needs to be a spark. In a love relationship, there is a gap between partners, a physical distance of individuality. When partners are open, the gap is viewed as an emotional space for which a spark of love can be exchanged. When sparks continuously fly, a relationship becomes alive and dynamic. It will become nourished and will grow; additionally, goodwill, trust, and happiness will flourish. However, when clogged and sullied by obstructions, the relationship will stall. Partners must regularly clean their connection; the space between partners tends to become obstructed by negative emotions, interrupting the flow between them. Often, when a relationship becomes disconnected, we hear comments like, ‘we just drifted apart,’ or ‘we outgrew each other.’ Frequent arguments are similar to the spark plug. Often, something gets in between partners, resulting in blame, pain, and misunderstanding. Like a wedge, the more these are inserted in the relationship, the more it drives partners apart. You might ask, “Why don’t couples remove the elements that

come between them?” Unfortunately, the wedge is not easily identified, leaving each partner to hold a distinct and separate piece of the problem. Instead of a clear gap, painful emotions, such as resentment, hurt, fear, insecurity, and avoidance, become stuck, causing a disconnect between the partners. While uncomfortable and hoping for resolution, each is a separate being, and neither has full control of the connection. When this occurs, negative emotions come between them, making agreeing and coordinating an effort to resolve the conflict very difficult. Typically, because each partner has a different solution, blaming each other adds another obstacle. Of that which comes between two lovers, the list is long: anger, resentment, frustration, disappointment, hurt, distrust, fear, discomfort, powerlessness, hopelessness, loneliness, the need to be right, self-hate, it goes on and on. It is this type of ‘dirt’ that clogs the connection. When calling in the “relationship mechanic,” the tool of teaching openness is one of the first steps in cleaning the connection. Next is the lubrication of the connections through teaching support and closeness. Finally, the necessity of investing in preventative maintenance is explained and practiced by building a new and refreshing relationship that will allow the sparks to fly.
