Should you visit the Emergency Room for HEADACHES? by Nesreen Suwan, M.D.

When headache medication fails and other treatment modalities no longer alleviate pain, how does a severe headache sufferer know what to do and when to visit a local emergency room? Patients may take different approaches to deal with intractable headaches that range from ignoring to overreacting. While it’s tempting for some to ignore headaches, put on a brave face, juggle household chores, handle work responsibilities, and taking care of everybody except yourself; it’s important to remember the important warning signs and red flags. The headaches that should not be ignored include and not limited to worst headache in your life, and first/ worst headache. With those you should seek medical care immediately. Those headaches might be due to a bad migraine or Cluster. However, it could be something far more serious like a ruptured brain aneurysm that may lead to dangerous bleeding in the brain. 8

Hyatt Magazine/ Issue 31

As such ignoring headaches is not recommended. ER visit is recommended for those types of headaches. Other types of headaches that should not be ignored include and not limited to headaches that are associated with other neurological symptoms like blurred vision, weakness, numbness or changes in mental status, fever and neck stiffness. On the other hand going to the ER for every treatment resistant headache/ migraine, seeking pain relief is not a good idea as well. In the ER order of priorities, headache / migraine sufferers are the last to treat, related to the fact that most headaches are not related to serious conditions, considering the significant life threatening conditions that a local ER could be flooded with such as stroke, heart attack, and traumatic injuries. It is also an unfortunate and known fact that emergency room staff may view headache/ migraine patients as drug seekers. After a long wait time in the ER, you may end up with strong

pain medication that will wear off in few hours, leaving you with either rebound headache or hang over type symptoms. Let alone the huge expense of ER visit that should be paid by you and your insurance. As such, visiting emergency room for pain relief may not be the best option. Intractable headaches may be difficult to treat as well, in a busy general practice setting. Considering the fact that there are so many types of headaches, and the potential seriousness of some of them, an extensive and specialized neurological evaluation, follow up and management is clinically warranted. Additionally, the field of headache medicine is rapidly growing, with many evolving treatment options that are available in specialty headache clinics. As such a specialty headache clinic may be the best solution for headache management.
