The burden often falls on the family to recognise the signs that an aging parent might need help with their activities of daily living to stay safe in the home. The standards are if a person cannot effectively do any one of these activities of daily living then they need care and assistance: 1. Eating – difficulty in eating or consuming nourishment. 2. Dressing - putting on and taking off all necessary items of clothing including medically necessary braces or artificial limbs. 3. Bathing - washing oneself on a routine basis in the tub, shower or by sponge bath. 4. Toileting - getting to and from, on and off the toilet, keeping a reasonable level of associated personal hygiene. 5. Ambulating – difficulty in walking with or without the assistance of a mechanical device, such as a wheelchair, braces, a walker, a cane or other walking aid device. 6. Transferring - the ability to move in and out of a bed, chair, wheelchair, with or without the use of equipment such as a cane, walker, crutches, grab bars or other support devices. 7. Continence - the ability to voluntarily control bowel or bladder function or, in the event of the in-ability to voluntarily control bowel or bladder functions (i.e. Incontinence), the ability to maintain a reasonable level of associated personal hygiene, with or without the use of medical supplies, equipment or devices such as protective undergarments or catheters.

NEVADA TEST SITE WORKERS BENEFIT FROM NUCLEAR CARE PARTNERS The Energy Employees Occupational Illness Compensation Program Act (EEOICPA), first implemented by the U.S. Department of Labor (DOL) in July 2001, provides compensation and medical benefits to former Department of Energy (DOE) employees who became ill as a result of working in the atomic weapons industry. The Program compensates those suffering from more than 20 types of cancers or other ailments including beryllium and asbestos-related ailments and chronic obstructive pulmonary disease (COPD). Uranium miners, millers, and ore transporters are also eligible for benefits if they have developed an illness as a result of toxic exposure at a facility covered under Section 5 of the Radiation Exposure Compensation Act (RECA).
Burt Stephens, an NCP patient, has had to deal with exposure to radiation. His story is both informative and inspiring. Burt joined the Army when he was 16 and was paid $50 per month. For an extra $50 he joined the paratroopers and started jumping out of planes. He was one of 2000 men who parachuted into the Korea conflict. Later, Burt worked in the Air Force where he worked on nuclear missiles and finished his career at the Nevada Test Site where he was an engineer technician who wired the bombs that were detonated at the site.

Advocacy & Assistance to frail seniors living in the community – Advocacy, assistance, information and referrals for Seniors who are 60 years of age or older. Elder Protective Services – For persons 60 years old and older who may experience abuse, neglect, exploitation, or isolation. Home and Community Based Services Waiver (HCBSW) For the frail elderly provides nonmedical services to older persons to help them maintain independence in their own homes as an alternative to nursing home placement. Personal Care Assistance Services (PAS) This program provides community based, in home services to enable adults with severe physical disabilities to remain in their own homes.
State Health Insurance Program (SHIP) Certified counselors provide unbiased information to Medicare beneficiaries, their families, and caregivers about Medicare and related health benefits. Senior Medicare Patrol (Nevada SMP) This program educates Medicare beneficiaries, their families and caregivers on how to: protect themselves from medical identity theft, detect errors, fraud and abuse, and report suspicious billing practices. Taxi Assistance Program (TAP) Program provides discounted taxicab fares to seniors and persons with disabilities based on income eligibility guidelines.

Honor Flight Southern Nevada Our mission is to honor as many of these remaining “ordinary people who saved the world” as we can by providing them an organized and all expense paid, escorted three day trip to the memorials dedicated to their sacrifices. Our volunteer guardians pay their own way, and assist those most in need throughout the trip. *Guardians must apply and receive training for the flight.

AdvantagePlusCaregivers.com’s “Transitional Care Services” We have seen so many our Care Recipients lose so much of their recovery from the Acute and Sub-Acute environments to the home due to lack of a good “transitional care” process. By the time the medication, supplies and the therapy gets into place after discharge so much of what they have gained in their recovery is lost. This is just unacceptable to us so we have created our own "Transitional Care Service" This service will not only help make for a smooth transition to home but will be a huge part of preventing readmission's that ultimately provides better out comes from the overall quality of care, reducing anxiety, stress, and other outside and unnecessary costs and delays in the process over all.
Our Transition to Home Processes and the Transition Team Transitional Care from us starts the moment an attending physician of a care recipient in an acute or sub-acute facility signs an order to release the care recipient home. We have established solid relationship with multiple Home Health Agencies, Hospice Companies, Geriatric Care Managers, Consulting House Call Doctors, when skilled care is needed. We have relationships with medical equipment and supplies, non-medical and medical transportation along with local pharmacies. 56
What is involved in the Preparation of the Transition? • We first prepare for the discharge by utilizing our simple one page transitions check list that with the discharge department of the facility we ensure the collecting the needed patient data: the diagnoses, care plan, doctors’ orders and instructions, medication list, supplies and equipment needed, important contacts, HIPPA release and advance directive (if they have one). • We meet with the patient, family members and anyone that maybe involved the patients care. • A home safety and aids inspection (time permitting) in preparation for the patient to come home. • Educate the care team, and schedule the appropriate caregivers. • The transition form is then signed off by the discharging facility representative. • Transportation for non-medical or medical pick up will have been coordinated by our Transition Team prior to the discharge unless other arrangements have been made by the patient or their care team. • The patient and their personal items are then transported home. • When the care recipient arrives home we make every effort to ensure that there care team (family members and loved ones) have been oriented by the Transitional Care Manager that includes the care plan, medications, and expectations. • The transition form is then completed by us and sent back to the facility confirming with the facility that the transition was successful.

How to Select an Assisted Living Home/Center The following is a consumer checklist of important services, amenities and accommodations in assisted living communities. Every community is unique. We recommend making several visits, at various times of day, to each residence you are considering.
Consider the following as you assess an assisted living residence: Atmosphere As you arrive at the residence, do you like its location and outward appearance? Did you receive a warm greeting from staff welcoming you to the residence? Does the administrator/staff call residents by name and interact warmly with them as you tour the residence?
Are doorways, hallways and rooms accommodating to wheelchairs and walkers? Are elevators available for those unable to use stairways? Are hand rails available to aid in walking? Is the residence clean, free of odors and appropriately heated/cooled?
Do residents socialize with each other and appear happy and comfortable?
Does the residence meet local and/or state licensing requirements?
Are you able to talk with residents about how they like the residence and staff?
Does the residence have a means of security if a resident wanders?
Do the residents seem to be appropriate housemates for you or your loved one? Are staff appropriately dressed, personable and outgoing? Do the staff members treat each other in a professional manner?
Needs Assessments, Contracts, Costs & Finances Is a contractual agreement available that discloses healthcare and supportive services, all fees, as well as admission and discharge provisions?
Are the staff members that you pass during your tour friendly to you?
Does this process include the resident, their family and facility staff along with the potential resident’s physician.

Individual Unit Features Do dining room menus vary from day to day and meal to meal? Are different sized and types of units available? Are units for single and double occupancy available? Do all units have a telephone and cable TV and how is billing handled? Is a kitchen area/unit provided with a refrigerator, sink and cooking element?
Social & Recreational Activities Is there evidence of an organized activities program, such as a posted daily schedule, events in progress, reading materials, visitors, etc.? Are residents’ pets allowed in the residence? Who is responsible for their care? Does the residence have its own pets?
Food Service Does the residence provide three nutritionally balanced meals a day, seven days a week? Are snacks available? May a resident request special foods? Are common dining areas available? May residents eat meals in their units? May meals be provided at a time a resident would like or are there set times for meals?
Services Can the residence provide a list of services available? Does the residence provide housekeeping services in residents’ units? Does the residence provide transportation to doctors’ offices, the hairdresser, shopping and other activities desired by residents?

The entrance to the walkway leading to the plaza, will be flanked by two fu dogs. The walkway lined with soldier palms, will lead to the Garden of Eternity Plaza. This feature will offer a place for ceremonies and will have ceremonial features added to enhance its use.
Memorials in the garden will consist of both upright and flat memorial options. Private and semi-private interment options are also available.
Garden of Eternity has been surveyed by local Feng Shui Master Lin Huang, trained at the Taoist Temple in Taiwan, so that it meets the laws of Feng Shui.
— Annual Events — In celebration of traditional Asian customs, the Chinese New Year and Ching Ming, and other special days of observance will be held at the Garden of Eternity.
Fu dogs known for offering a symbolic protection to a property, will be placed at the leading walkway of the Garden.
The garden is positioned in the South Eastern area of the cemetery and will overlook Legacy Lake and have a clear view of the Las Vegas mountain range to the Northwest. This arrangement will best benefit from the Yin manifestations of quiet, darkness, moonlight, and stillness—resulting in a positive response from visiting friends and loved ones, who experience the compatible aura.
