WELCOME

FACING A CRISIS

W

e’re not going to sugarcoat the truth, distort reality, and as many will attest to,

we won’t publish fake news. Integrity and a willingness to uphold legitimacy in reporting—at the most critical junctures of daily living, is our mission. We follow and analyze the 24hour news cycles, so we may present a straight and unadulterated point of view to our readers who demand a full and accurate account of stories relevant to their lives. With this obligation comes intense pressure to provide honest and historical accounts of important events within a specific duration of time. Our job is to expose injustice or hidden agendas that affect us all. Adhering to this premise, is it any wonder why we are sometimes the polar opposite of what the mainstream media preaches as gospel to the unsuspecting low-informational consumer (or radical) political partisans? Much of our efforts point to the distortions and the manipulations of facts. We attempt to expose a deceptive media who consciously influence populations 10 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

and elections by feeding the public inaccurate—even false accounts—of the news, events, and orchestrated perspectives produced by political parties, for delivery by news sources daily. We are serious about our work and equally committed to reporting what we see as dishonesty and insincerity by those who are perpetually hungry for power. But, fear not, there is a growing stable of organizations willing to peel back the façade so you might have a clear view of the facts and enjoy an alternative point of view. Unlike many in today’s media, we don’t overlook questionable content, play to political parties or appease to those who peddle influence; instead, our mission is to tell a story of our time and the events that shape us into who we are at this moment. This month, readers will find that we have documented many of the experiences citizens have faced, or will soon endure, as a result of the outbreak of COVID-19. You’ll learn that the Caronavirus— which originated in China, (regardless of what you are being fed by false news and social media outlets), has turned into a political football by an enemy of the people. These reprehensible individuals,

partisan groups, and some in the media have politicized and manipulated a worldwide health crisis. They have enhanced the financial meltdown of the stock market, and are attempting to create a distrust in the federal government. While wringing their hands over personality conflicts between themselves and the current Administration, they plot and strategize the most devious and divisive means of splitting the public, while following the infamous words of Rahm Emanuel, “…never let a serious crisis go to waste.” Oh yes, we face a crisis, and the first step is to search for the truth so that we might uncover those with self-serving agendas. It is unfortunate when the country needs a unified voice from its leadership; it gets discourse, loathsome accusations, and false innuendos. Our April issue provides a front-row seat to view the lack of a cohesive and unified voice coming out of Washington. It also serves as a guide to understanding how disruptive the spin—pumped out around the clock—can fog our vision, and how some will use dishonest tactics at the expense of our nation’s health and economy. ENOUGH IS ENOUGH

E

veryone has a right to their opinion; unfortunately, sometimes beliefs spill over, resulting in the casting of blame onto others for a host of reasons. Case in point; have you ever had a furious argument with a personal friend, husband, wife, or child? Affirmative. So, try to recall, did the discourse get resolved by yelling, screaming, and blaming the other person, or did reasonable minds come together, and an adult discussion followed? Probably. Any good therapist, arbitrator, or mediator will tell you that until you arrive on an equal platform and weigh the strengths and weaknesses of the opposing party’s arguments, nothing can be resolved. Also, in most negotiations, no one achieves 100 percent satisfaction or is entirely correct, gray is the color of compromise—not red or blue. With this knowledge, let’s tease out some facts from the misinformation surrounding the Coronavirus, or more accurately termed, Severe Acute Respiratory Syndrome Coronavirus 2, shortened to SARS-CoV-2. FACTS: ➤ The virus originated in China, and their

leaders stalled in releasing a warning 12 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

about its existence at the first stages of its moving from animal to human. ➤ The disease is highly contagious, can cause serious illness—even death within specific populations. ➤ We face unknowns, but the administration and citizens are attempting to slow down the transmission of the disease in the US. ➤ The President, upon learning of the disease’s effects, sealed borders and prohibited travel from regions that could have caused US citizens serious injury, only to be criticized for his timely and now appreciated insight and response. ➤ The media—specifically, Jim Acosta, CNN’s chief White House correspondent (who has had multiple verbal altercations with President Trump over the last three years), stated that a comment made by President Trump during his Oval Office address, “smacked of xenophobia.” Acosta’s comment referenced the President calling the Coronavirus a “ foreign virus.” ➤ The mainstream media allowed a personal attack—when on February 26, 2020, New York Times, Opinion Columnist, Gail Collins, penned a piece, “Let’s Call It Trumpvirus, with a subhead; ‘If you’re feeling awful, you know who to blame.’” (Interestingly, when researching for this information, Google Chrome didn’t make

the hit after multiple attempts, while Firefox found the target information on the first try). ➤ On the same night—and on the same cable station—where Jim Acosta made a false accusation, (see above), CNN contributor Don Lemon got into a heated exchange with former Governor and CNN analysis John Kasich for not condemning the President’s Oval Office address. Here is a copy of the transcript: DON LEMON: You said the President should stick the script. I don’t know if he stuck to the script or not. But whatever script he read was wrong because they’ve had to clarify it several times. And I just got to say if the President came out to calm people’s fears, he didn’t do a good job of it because they’ve had to come back and clarify it several times. And this has been going on long enough for them to get it straight. We need straight, accurate information from this President and this administration. And I don’t understand why you are tiptoeing around it. He came out. Gave an address that usually—that happens very rarely. And he doesn’t get it right? KASICH: I’m going to tell you–first of all, he read it. Look, I don’t want to get into that. He—

LEMON: Why not? That’s why you’re here. To talk about the President. KASICH: Can I finish now? LEMON: No, you can’t John, because we are here to talk about the President’s— KASICH: I can’t talk? LEMON: We’re here—I don’t want you to go on and deflect and talk about something else because we’re here to talk about the President’s address, and you said that someone else wrote it. He’s the President. Even if someone else wrote it, it should be right. KASICH: I’m trying to say to you Don that we got to move down the right. Looking back doesn’t get things fixed. LEMON: We’re not looking back. The President’s address was tonight. That is the newest information. KASICH: I think his address tonight. I think it was fine. I think it was fine. That’s what I think. LEMON: You think the wrong—you think inaccurate information that you have to clarify two or three times, you think that’s fine? KASICH: It was fine. It was the right tone. I thought he did fine. That’s what I thought. I thought he had the right tone. And he no longer played around with we’re going to blame Democrats or we’re— forget this stuff. He—I hope he’s trying to move forward because he understands the seriousness of this situation. Now, the fact that they clarify, it isn’t unusual in a speech for somebody to clarify something. Okay? But I don’t think that the tone he showed tonight takes away from some clarification. I think he set a serious tone. That’s what I wanted out of him for a long time. And I think we got it. LEMON: John.

KASICH: That’s what I think. LEMON: I can be serious and sit here and tell you four plus four equals 85. I am serious. That doesn’t mean that I’m right. The information he gave was not right. You have to come back and clarify several times, information that you gave. Listen. I tried to cut him a break at the top of the show. KASICH: Like, who’s going to get back into the country? You mean—what is it the clarification that you thought was so terrible? Tell me what you thought. LEMON: I didn’t say I thought it was terrible. I’m telling you that we are in the middle of a pandemic right now. We’re in the middle of a pandemic. And every time this President comes out, he gives inaccurate information. So I don’t understand why he would even come out and do it at all if he is going to confuse the American people even more. You don’t understand that? You’re a governor. I’m surprised that you don’t understand that. If you’re not going to help—if you’re not going to be accurate, why do it? KASICH: No, Don, I do—no, I do understand it. Look, I’m just trying to tell you that I thought his tone tonight was good. So he had to clarify something. Okay. 98 percent of what he did was good. And what we wanted was a seriousness out of the President, to tell people that things are not good and they’re probably going to get worse. And I’m not saying this is going to disappear or go away. This is very serious stuff. Now, if you got to clarify, are you still going to let products come into the country, okay, I wish it didn’t have to do it. But it doesn’t take away from what I thought was the seriousness of the matter. Because we all have to understand how serious it is. And I’m not deflecting or running away. ➤ And, of course, there is the unnamed

source who felt it necessary to offer an opinion of the situation, when they were quoted in TheHill.com; “I’m not sure this administration has the intellectual capacity

or the strategic understanding to address this,” said a former State Department official who worked under the Trump administration and previous presidents. “I really do think this could end up being Trump’s Katrina, where people finally begin to question his ability to govern.” Much of this sounds repetitive, and while everyone understands the level of anger felt by Democrats, we would hope they would take a moment to consider the harm their actions cause. The country needs to unite and work together for the common good of humanity; there are numerous factions which seem to indicate they would prefer to see the breakdown of society and live through the horrors of civil discord—even anarchy—with the intent of a new administration in November. It is objectionable that anyone would attempt to undermine a sitting President, and his administration midst a worldwide crisis. It isn’t as if nothing has been accomplished; every safeguard, proposal, and fiscal action has been implemented in a timely fashion. These actions are in addition to meetings being held and efforts cemented between the government and the private sector. It will not be an easy road for anyone, so shouldn’t we be the cheerleaders of this great country, rather than looking like idiots to the rest of the world because of infighting? This commentary began with the notion that everyone has an opinion. But, it has evolved into a request that politicians and the media provide citizens a balanced voice so they might evaluate progress made and sincerity. We advocate for the people, and now is the time for both parties to do the same. Stop the antics and power grabs, cease the deceptive practices visible to a voting public, and end the hostilities that make this fight so unbearable and offensive to watch. In November, the people will speak, and their word will be the final evaluation on how well our elected officials carried themselves through a crisis no one was prepared to handle. ecause of the unfortunate and untimely arrival of the Coronavirus, we have begun to learn about social distancing, physical isolation, and—quite possibly—the need to retreat to a safe and secure location. By following these steps, one can significantly lower the chances of contracting the modern-day plague while also offering a quick and easy means for mitigating contaminants. Caravans of New Yorkers are headed to the Hamptons, for the reasons above, simultaneously, we’ve noticed an uptick—or at the very least, an earlier than usual arrival of homeowners to the hidden summer enclaves of the SouthCoast. The timing is interesting and gives rise to the question, are

homeowners seeing the value of owning along the coastline because it offers unique protection from high-density areas? Perhaps. This transition makes sense because if you live in a populated urban location, your risk factor quadruples. The majority of cities don’t allow for six feet of space around an individual, so heading to such a beautiful place to be able to live freely is not only desirable but a wise decision. Having an exclusive compound to escape doesn’t have to be a response to a pandemic; it is useful and serves a multitude of purposes. Primarily, it is an ideal environment to relax and enjoy summers and holidays and the shoulder-seasons with family and friends. Plus, having easy access to Buzzards Bay and a long list of sailing destinations of the northeast, the harbor of Padanarama is the perfect home base. NEW ENGLAND NEWS + MEDIA | APRIL 2020 | 21

This month we are pleased to review a stunningly beautiful home located within the confines of Birchfield Farm in South Dartmouth, Massachusetts. A very private location, the home sits high above the flood-line and across from Ricketson’s Point and the harbor’s breakwater. The featured residence with its dramatic roof lines, bluestone terrace, a three-season enclosed patio—with an outdoor fireplace—offers 5,424 square feet of elegant and luxurious living space. Thoughtfully placed on nearly two acres of rolling lawns, complemented by stone walls, it is a shining example of how other private properties are measured and evaluated for their value and pleasure. Being relatively new, (constructed in 2005), but steeped in tradition, the home has four bedrooms, four full baths and two half-baths with exceptional features and appointments. An open-space great room combines a living room, dining room, and brilliantly designed kitchen, which offers a marvelous space for preparation or entertaining. The form and function of this central location are grand and inviting. The home’s interior boasts extraordinary craftsmanship and attention to detail. From a mahogany domed ceiling located on the second floor in the observatory to the custom countertop with wet bar reminiscent of a yacht’s galley in the dining area, these unique details provide a personalized feeling to the environment. Attention to detail is a vital feature in a home of this class. It is seen in the Brazilian mahogany flooring, two fieldstone fireplaces, a beautiful mahogany mantle, expansive coffered ceilings, a quiet study with custom built-in shelving, and cabinets, and the stunning light and views found in all locations. The community is fabulous and allows one to be as private or social as desired. The intimate neighborhood is complimented with like quality homes and is laced with walkable roadways in and around the compound; the setting exudes freedom and peace. The home is in like-new condition and is ready to be moved into by a family looking to begin a legacy. The tasteful interior would be a perfect palate for a wide range of tastes and is ready for new owners to display their collection of art and furnishings. 12 South Lane at Birchfield Farm is one of the most impressive and complimented homes on Padanaram Harbor and is always a welcoming sight to mariners who head to the yacht club or to pick up a mooring. Offered at $2,975,000, listing agents Will Milbury and Jeanne McGlone would be pleased to schedule a tour of the property and provide additional information about this one-of-a-kind retreat regardless of the reason you’re searching for glorious and private living. Common Sense & Education Will Lead to the Death of the Covid-19 Virus BY MARGO ROBERTS

I

f you’re reading this feature, there are a few assumptions to be made; (1) you’re online enjoying the April issue—in its digital version, and we thank you for taking this step. (2) Our magazine was delivered to your mailbox and carefully extracted from the protective plastic wrapper. Or, (3) you picked up an issue at one of our many drop-off locations where our titles are found in large bundles. Finally, and the least desirable choice is that you picked up an issue at an office, coffee shop, or restaurant that allows guests takeout options. In the last two instances, our magazines are packaged and protected using clean techniques, and delivered in a secure wrapper; however, once exposed to the air in public places, the chances of the virus showing up on the covers are increased. 28 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

We’ve been in contact with those involved with our production; this includes writers, photographers, designers, printers and, delivery staff to discuss our processes, so our publications get into your hands absent of contamination by aerosolized particles of the virus. Our periodicals are wrapped in heavy plastic and are completely safe. However, once put on display, they are subject to environmental hazards, the same as the coffee cup you’re drinking from, the bottle of water pulled from a cooler, or the plastic utensils picked up from your favorite restaurant. It comes down to this; if you didn’t unwrap it with your clean paws, then calculate that it is infectious and needs to be cleaned before use. Research shows that Coronavirus can spread quickly. For example, the ATM at

your bank, the mail delivered to your home, the door handle to any of the dozen or so places you visit, even the coat you’re wearing in public today, all could carry the virus. It isn’t a joke, the risk to our health is heightened because the life of the live virus (whose droplets are invisible) is hours— even days—putting us at an increased risk if we do not take all necessary precautions. Most individuals are too busy, or they don’t realize that everything they touch during the day is comparable to licking the surfaces other’s hands have touched. Of course, many businesses have upped their game and are regularly cleaning, but this isn’t fool-proof, and that is proven by the rising contagions’ numbers. Once you’ve touched a surface and put your hands to your mouth, nose, or eyes, you will have introduced the virus into your

body—which serves as the perfect host— and allows for incubation (growth); all while you are unknowingly infecting others. Because the virus is microscopic and not visible, many come to the false conclusion that the Coronavirus isn’t in their neighborhood. Unfortunately, this silent and unobserved traveler is around you. Unless you’ve been isolating for the last two weeks or have taken extreme measures—such as wearing a mask, gloves, and eyewear while in public—you could be carrying the virus and sharing its possible deadly effects. It’s a personal responsibility to prevent the spreading of Covid-19, but this cannot be done unless we all make a conscious effort and restrict our contact with contaminated surfaces and keep at a reasonable distance from others; vigilance is the key to success. BY JAMES HOLDEN

oom and gloom, it’s everywhere you look. Masks, gloves, and maybe soon, hazmat suits may become the fashion of the day. Early adherence to the early warnings would have been a wise choice by packing the house with all the essentials and using what precious time you had available. And, once you had everything in place, you could contemplate your investments and enjoy the free education about economics and the market available on thousands of websites. For first-timers, we’ll give you a break, the whipsaw action of the market isn’t kind to a newbie’s psyche and could feel more like a baptism by fire, but still, there are lessons to be learned for a lifetime. First and foremost—DON’T SELL ANYTHING! Losers panic, while the smart money plans. There is much to be learned in how to evaluate the economy by reading, listening, and watching the best and brightest in the financial industry. It’s also important to keep emotions out of financial decisions. It’s a strategy that will serve you well. During the last major market crash (September 29, 2008), I had bailed on a portfolio of stocks just in time to avoid the pain. Some call it timing, while others claimed it to be luck. I remember asking myself two questions before the meltdown; is owning a home a right or a privilege, and does everyone deserve credit regardless of their financial history? The answer was easy to calculate, and I saw the writing on the wall and realized we were going to slam into it very soon. The banks and mortgage lenders would give anyone a pulse a loan, and as for down payments—what were those, they seemed unimportant and were overlooked. So, you might wonder what I did with the cash I was sitting around. First, I invested in real estate, paid off mortgages, and sat on the rest of the money for a long time. The problem I ran into was I became distrustful of the market and missed the run-up of the bull-market; albeit, I slept well. I finally got back into equities, and once I found that the water was safe and I began to study and follow trends, I also went back and reacquainted myself with some of the new guys who were armed with analytic models that made some sound predictions about where the market was heading.

One important rule I learned from the Oracle of Omaha— Warren Buffet was to buy companies, not stocks, and hold on to them for the long haul. If you take a look at this advice, you’ll find that when the market melts, and a bear follows, it is the wise investor who dollar-cost averages, or at the very least, holds their positions and refrains from panic selling. Another great investor worth mentioning—and whom I am a big fan of—is investment rock star Peter Lynch of Fidelity Investments. One of the greatest mutual fund managers of alltime, Lynch earned an average of 29.2 percent return for his clients through the Magellan Fund between 1977-1990. What separates his ‘genius’ from others is his investment principle, “Invest in what you know.” This advice always made sense to me and resulted in my investing money into companies I understood, or at least, I could see they had created a revolutionary product or service. Today, we have to wonder what the future will look like; should we consider investing in space travel, driverless electric cars, or clean water? As far-reaching as these ideas seem today, for those genuinely interested in long-term investing, these concepts and the companies who will become leaders in their categories should be considered for portfolios of the future. Many seasoned investors are quietly maneuvering around the market, and so it may be time for you to start selecting solid stocks that you have an understanding of and buy them to own for the next five years. Think about global air travel, bio-medical treatments, longlasting battery manufacturing, and technology. The best investors are taking small bites from the apple, and even if they don’t get the lowest price on every purchase, they’re adding to their positions slowly and cautiously so that in the end, they have a dollar-cost average on their side. It takes a stomach of steel to buy stocks when they are falling like heavy rain, and this is when emotions need to be checked at the door. For those engulfed by fear, consider an ETF or mutual fund, which are excellent methods for attaining benefits of capital appreciation without losing too much sleep. As a contrarian, I see the cup half-full and great buying opportunities; how about you? H NEW ENGLAND NEWS + MEDIA | APRIL 2020 | 33

LOOKING GOOD Summer Will be Here Soon BY MARGO ROBERTS

W

ith the headlines buzzing about social distancing, we feel that offering a fashion plate showcasing attire that would never see the light of day would be impractical and a waste of paper and ink. The whole idea is not to be packed in with others. Plus, nearly all of this month’s (and perhaps, May and June), many galas, fundraisers, and soirees will have been canceled. (see our notice at the beginning of our Event’s section).

36 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

But, not all is lost; many of us will face the challenges the pandemic delivers with grace, style, and common sense; and, it doesn’t mean we can’t keep ourselves busy and looking good for the summer. This month we’re going practical, and so we’d like to share some observations and scientific advice we found online. With the temperature rising, people getting out of the house and into the wide-open spaces, (do keep six feet from your closest neighbor), and the sun beaming down on us, means that the Coronavirus doesn’t stand a chance. Walking the beach, relaxing by the pool, or heading out on the boat, means that you’ll be able to use this reprieve to relax and get some much-needed rest, while at the same time, those dangerous ultraviolet (UV) rays will be obliterating the virus. It’s true; sunlight is a critical defense in killing off harmful bugs!

NEW ENGLAND NEWS + MEDIA | APRIL 2020 | 37

STYLE

You might be thinking, “Okay, but what are those UV rays doing to my skin?” We’re glad you asked; first, buy some high-octane sunscreen—before a shortage—pull out your wide brim hat and oversized sunglasses and explore the stylish world of Cabana Life. Known for their UV protective clothing, company founders, Melissa and Allyssa, (both faired skinned), tell their story; “At the time of Melissa’s malignant melanoma diagnosis at 26 years of age, we were just like many of you, [we were] shocked to find out that our lightweight clothing wasn’t protecting us. We started looking for stylish sun protective options and realized they didn’t exist for ourselves and families.”

38 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

Keeping with our favorite design philosophy—form & function—the ladies created the most artful and vibrant sun protective clothing available. Using the highest-rated Ultraviolet Protection Factor (UPF 50+) fabrics, Cabana Life clothing not only looks fabulous, but it blocks 98 percent of UVA and UVB rays from getting to your body. Smart, classy, and gorgeous, Cabana Life offers looks and styles at affordable prices. Traditional cuts and designs, flatter and highlight all shapes and sizes. Being casual, their line of fabrics and looks will make you feel comfortable and give you pleasure during tough times.

INTIMACY

IT DOESN’T MAKE MUCH

SENSE BY ANDREW AARON, LICSW

W

hy is it that when an old problem is resolved, a resentful partner may grow angrier rather than happier? This defies logic, and one would believe that when a partner is given what he or she has wanted for years, gratitude would be expressed. Unfortunately, it isn’t always the case; often, the longer a chronic problem persists, the more pronounced this headscratching dynamic becomes. When problems mount, loving feelings get buried under negativity. The journey back to a loving connection faces headwinds. Like the truth in physical science, gravity makes it easier to fall rather than rise; the same for a relationship, more effort is needed to maintain a satisfying passionate relationship or marriage than is necessary for it to devolve into a poor one. In part, it is the long-standing unresolved problems generating a build-up of resentment. The partner who finally makes a change to eliminate a perpetual problem expects immediate and positive results, which would be formulated by appreciation and gratitude. But, it isn’t always the case; making positive changes happen only with significant effort, and it sometimes feels like it goes 44 | APRIL 2020 | NEW ENGLAND NEWS + MEDIA

against what comes naturally. When the other partner reacts by withdrawing, the motivation for continued efforts is discouraged, potentially pulling the couple back into disconnection. Many couples try to pull themselves out of unhappiness only to fall back into the pull of negative relationship habits because of this phenomenon. The pattern doesn’t seem to make sense; why would partner improvements not be rewarded? Uncovering the specifics gives us a fascinating look into how we as humans love and function. The truth is that if our behaviors, choices, or attitudes don’t make sense, then the source of it is emotional, and from our feelings. Within love relationships, people interact with emotions. Yet in our daily tasks, most of us function predominantly from our intellect, especially in our jobs. When perceived through the lens of the intellect, most emotionally-influenced choices and behaviors make no sense. More than most of us would like to admit, we act and live the way we “feel” like living. Though not valued as highly as sharp intellect, intense emotion is deceptively powerful. When a love relationship has been unhappy for years, the romantic partners are in pain. Each partner copes with grief in unique ways, but common practice is to repress the painful emotion to feel it less. Burying the feelings deep within, away

When a love relationship has been unhappy for years, the romantic partners are in pain. Each partner copes with grief in unique ways, but common practice is to repress the painful emotion to feel it less. Burying the feelings deep within, away from conscious awareness, helps unhappy partners endure their discomfort, especially in situations in which they are powerless to fix the problem.

Each lover has control over his or her behavior, but not over the other. The only power a resentful partner has is not to make problems worse while waiting for the awakening of love and compassion from the other in the form of a willingness to change old problem behavior. Upon the arrival of that moment, when the other partner takes a long-awaited corrective step, the resentful partner does not feel immediate relief. Instead, the appearance of loving efforts, and perhaps compassionate acknowledgment for the painful impact of chronic hurtfulness, arouses and brings to conscious awareness the painful repressed feelings.

This is not relief, but instead, the hurt and anger that has been buried are now felt. Emotional pain rises to the surface of conscious awareness to be healed, and as a result, a sincere apology may, unfortunately, be greeted with coldness. After improvements have begun, couples will eventually be rewarded if they hold on tight by persisting towards growth. Healing is not a linear or logical journey because emotions have their own non-rational “logic. Perseverance in loving will bring an end to the angry feelings soon enough. Love is infinitely patient when blocked; it always finds a way to be expressed. H Andrew Aaron, LICSW, is a sex and relationship therapist who practices in the New Bedford Seaport. LIVING WELL

Hope

for the severely depressed When medication fails, many find relief with transcranial magnetic stimulation

BY ROB SAINT LAURENT, M.Ed.

T

he Brain and Behavior Research Foundation describes 65-year-old Susan Burns as an extrovert with a bevy of friends and a driving enthusiasm for life. But, that was before a recent onset of severe depression, precipitated by a rash of deaths in her family, left the long-time registered nurse feeling like a failure at her job and without the energy or desire for the things she once enjoyed. Despite multiple antidepressant medications, including the drug Paxil, Burns remained a shadow of her former self while also experiencing significantly reduced sodium levels from the SSRIs (selective serotonin reuptake inhibitors) she was taking.1 Severe depression impacts about 350 million people globally and, according to the World Health Organization, is the leading cause of disability. The National Institute of Mental Health (NIMH) says that about 17.3 million or 7.1 percent of US adults experienced a major depressive episode in 2017, many with severe disabilities from their depression. Up closer, 5.3 percent were men and 8.7 percent women, with younger adults aged 18-25 having the highest prevalence. That year, some 65 percent of severely depressed adults received a combination of medication and mental health treatment, with another six percent medication alone.2 However, like Susan Burns, about a third of people with severe depression do not recover even after multiple medications.3 The human and economic toll is not insignificant.

PROMISING NEW THERAPY

Burns was left with two options at the suggestion of her psychiatrist: electroconvulsive therapy (ECT) or transcranial magnetic stimulation (TMS). ECT is an invasive method involving electricity applied externally to a patient’s head (typically 70-120 volts). Though effective, the practice has become stigmatized and has usually been reserved for the hospitalized. Burns instead chose TMS, an emerging safe and non-invasive therapy approved by the US Food and Drug Administration in 2008 for treatment-resistant depression, defined as an illness that fails to improve with at least one antidepressant medication. TMS involves a repeated series of on-off magnetic pulses emitted by what looks like a large dental apparatus. The patient reclines, and an electromagnetic coil is strategically placed against an area of the forehead believed to control mood (typically the left occipital lobe). Similar in strength to an MRI, the short pulses penetrate one to two inches into the brain, producing a slight tapping sensation on the head. This repetitive on-off pulsing lends to the treatment’s formal designation “rTMS.”4 The Mayo Clinic explains that a series of 20-30 treatments are given over a four to six week period, Monday through Friday, with a typical session lasting 40 minutes. Afterward, a tapering program of less frequent TMS sessions is usually undertaken. Regarding effectiveness, a 2012 study showed 168 of 300 patients (56 percent) experienced improvement in symptoms by at least 50

percent, while a 2014 study of 250 patients showed 63 percent of those feeling improved still doing well after one year.5 Anesthesia is unnecessary, and memory and alertness are not affected so that a patient can drive home. However, the machine can be loud, so earplugs or headphones may be needed. Side effects may include mild to moderate scalp discomfort and facial twitching or tingling during therapy, as well as brief lightheadedness and treatable mild headaches. That said, the possibility of seizure exists. NIMH says recorded instances are rare (fewer than 1 in 1,000). But Mayo Clinic advises that people with a history of seizures or implanted “ferromagnetic materials” (example, nickel, iron, cobalt) within a foot of the magnet, avoid TMS. NIMH also states that since the therapy is relatively new, “long-term side effects are unknown.”4 THE BRAIN ON TMS

TMS creates a weak electrical current in nerve cells of the targeted brain area, which, it is believed, either raises or lowers cell activity to restore healthy balance and mood. Daniel Press, MD, Clinical Director of the TMS program at Beth Israel’s Berenson-Allen Center for Non-Invasive Brain Stimulation, says the physics behind TMS has been known for about 150 years. When a powerful magnetic field is quickly turned on and off, an electrical current will be created in any wire running perpendicular to it. The current is weak enough that it does not hurt the brain but strong enough to get the nerve cells to “fire.”6 But, there has been little research on the actual goings-on in the brain during TMS. In one recent study in the journal Proceedings of the National Academy of Sciences, German investigators showed that TMS reorganizes the “orientation” maps in the visual section (occipital lobe) of the brain, an area partly influenced by both genetics and the environment. After high-frequency TMS, the researchers found neurons seemingly open to new tasks, creating a potential window

to introduce added therapy (in this case, visual training) for the neurons to change their connectivity pattern.7 The jury is still out as to whether TMS works best alone or in concert with antidepressants and/or other therapy to relieve depression. There is also disagreement among scientists regarding optimal magnet positioning and pulse administration.

PRACTICAL EXPERIENCE

On the SouthCoast of Massachusetts, child and adult psychiatrist Jonathan Schwartz, MD, offers TMS therapy to patients with complaints of severe depression. Offering a range of treatment options, Dr. Schwartz does rely on TMS in his practice; he offered his observations of the treatment. “Transcranial Magnetic Stimulation (TMS) is a non-drug therapy approved 12 years ago by the FDA for the treatment of depression that has not responded to multiple antidepressants. TMS uses magnetic energy to directly stimulate the area of the brain that is known to be important in depression. This treatment has been proven to be both safe and effective. Treatments last about 18 minutes for most people, with up to 36 treatments for a complete round of TMS. [We’ve found], many patients can stop taking antidepressant medications after successful TMS, [albeit], some continue on medications.” MAKING SHINY PENNIES

While TMS doesn’t work for everyone, about 30 percent of patients experience a full reduction of depression symptoms. According to Schwartz, 70-80 percent experience either complete remission with at least 50 percent improvement in their depression. For Susan Burns, she says she once felt like a “dull penny that had been in circulation for many years,” yet related to her doctor that she wanted to be one of the “pennies that are shiny and look brand new.” She seems to be on her way to achieving that goal, as she is now in remission and is regaining the ability to experience pleasure and the relational intimacy she once enjoyed. Burn’s side effects were minimal, like that of many other TMS recipients. And while she has continued her medication regimen, she recognizes that it was TMS that made a noticeable difference. The treatment is pricey, based on length and number of sessions needed, but “TMS is paid for by virtually every commercial insurance in the country,” noted Schwartz. More information on insurance and TMS locations can be found at neurostar.com. H

Locations: Sugar Hill NH-Carolina Crapo Building and Sugar Hill Town Meeting House

Rosecliff Mansion, Newport, RI New England’s Premier Flower Show

Every year Sugar Hill and surrounding areas celebrate the season of Lupine. For about a month, you can see the lupine flower blooming all over northern NH. There are also several bands and musical performances throughout the weekend, as well as educational talks and events to ensure there is something for everyone attending the market.

Voices are like fingerprints. Each has a unique register and leaves an indelible signature. A garden is the reflection of the gardener’s voice. Whether that voice echoes the strength in mighty trees, the romance of fragrant flowers or the laughter of bubbling water is still heard in great gardens.
