ecently Dr. Schilling was privileged to attend a private course with Zein Obagi, MD, a world renowned dermatologist and formulator in Beverly Hills, CA, who developed Obagi Skin Care Products.

This course was not just about “selling” Obagi products, but providing the best available treatment to people who desire improvement in their appearance at any age. Some of the topics addressed were prevention, skin function, and maintenance to keep skin healthy, young-appearing, and free from deteriorative signs of aging and skin disease. The objective of the course was to help physicians like Dr. Schilling to use a systematic approach to restore skin to its natural, healthy, youthful state. Healthy, youthful, attractive skin is the goal both patient and we at Schilling Medical Spa strive for, and it should be possible for persons of ALL skin types and colors to achieve, whether through the Obagi Skin Health Restoration program alone or in conjunction with a procedure. Sometimes, physicians become skilled in one procedure, be it chemical peeling, laser treatments, or surgical intervention, and then use that procedure for all patients indiscriminately. Dr. Schilling agrees with Dr. Obagi, each patient is an individual with personalized needs. The indiscriminate use of the same procedure for every patient-- should end. They are not necessarily better and may be worse than more limited procedures. Procedure selection should be based on the needs and realistic goals the patient wants to achieve. hile Botox parties can be perfectly safe, as long as the administration of Botox is conducted by or under the supervision of an experienced physician in a sterile environment, there are some surprising risks that stem from just joining in the fun that you may not be aware of as a participant.

With the popularity of Botox, it is natural that people would offer discounts, knockoffs, and innovative marketing approaches. Botox parties are part of that: women and men are finding that they can get a facial rejuvenating treatment along with hors d’oeuvres and a glass of Merlot at a price comparable or cheaper than their local cosmetic surgeon’s office.

A host or hostess of a Botox party may be familiar with the injector’s experience, as a guest, you may not be. Botox, as well as cosmetic fillers, are increasingly available, and it’s possible the person administering Botox at a party may not even be a licensed professional. Another consideration, even if your injection went smoothly, many times patients desire a touch-up or need a follow-up visit after the injection of Botox or Filler, where will you go and who is responsible for your results?

Cosmetic surgeons are becoming more and more concerned about these injections being dispensed by unqualified and unsupervised individuals. Word of advice, calculate your risk before you consider the price.

We asked Dr. Schilling to answer some of the most common questions related to female issues he has been asked over the last 30 years in practice. Although each woman has different circumstances, we all can benefit by learning from each other. Please feel free to make an appointment to discuss your individual problem or question.

Dr. Schilling ANSWERS QUESTIONS about... the MOST asked about female issues.

I leak urine when coughing or exercising, can something be done? Urinary incontinence is common in women and all types of urinary incontinence can be treated. It can happen at any age, so you don’t need to be embarrassed. There are a number of reasons for leaking urine and a full evaluation is done in our office to determine the diagnosis. If stress urinary incontinence is the cause, I can offer and perform an outpatient urethral sling procedure using a synthetic mesh material. What can I do for this constant bleeding all the time? This is one of my most common frustrations complained about by my patients. Unfortunately, there is not an easy one-for-all answer. Abnormal or dysfunctional uterine bleeding has many causes and treatments. An office evaluation, pelvic exam and sonogram of the uterus are required. Treatments are individualized and sometimes include medical therapy, endometrial ablation and hysterectomy depending on your circumstances, desires, existing health issues, and age. What is endometriosis? This seems to be another common issue of my patients. Endometriosis is an often painful disorder in which tissue that normally lines the inside of your uterus — the endometrium — grows outside your uterus. Endometriosis most commonly involves your ovaries, bowel or the tissue lining your pelvis. Rarely, endometrial tissue may spread beyond your pelvic region. In endometriosis, displaced endometrial tissue continues to act as it normally would: It thickens, breaks down and bleeds with each menstrual cycle. And because this displaced tissue has no way to exit your body, it becomes trapped. Surrounding tissue can become irritated, eventually developing scar tissue and adhesions — abnormal tissue that binds organs together. This process can cause pain — sometimes severe — especially during your period. Fertility problems also may develop. Fortunately, effective treatments are available. What is HPV? HPV stands for Human Papilloma Virus and is known to affect 25-35% of the population. It is transmitted sexually among heterosexual and homosexual persons. Four of the approximately 40 subtypes have been implicated in causing cervical cancer which is why you need to obtain PAP smears. Newer DNA technology allows PAP smears to detect the presence of HPV. Doctors can treat the cellular changes associated with HPV but not the virus itself.

What are my options for permanent birth control? You have several options, the most common known to most women is a tubal ligation procedure. This involves destruction or blocking of the mid-portion of the fallopian tube so that the sperm cannot reach an egg. Some are performed surgically by placing clips on the tube or removing a portion of the tube itself. In recent years we can offer more options for permanent birth control, the Essure™ or Adiana™ procedures are performed in the office under local anesthesia without any scars or downtime. The actual procedure only takes 8 to 15 minutes, you should be in and out within an hour. The success rate is excellent and patients seem to prefer the office procedure over the hospital. Most all insurance companies cover permanent birth control. Do hormones cause breast cancer? It has never been scientifically shown that estrogen hormone administration CAUSES breast cancer. Other scientific studies have demonstrated a slight increased risk of breast cancer. I am very careful to listen to your specific needs, family history, and previous gynecologic history. After carefully discussing the risk and benefits I may prescribe hormone therapy as indicated for specific menopausal symptoms and treatment and will always individualize each treatment plan for each specific patient. Do I need a PAP smear after a hysterectomy? Probably not IF you had no prior history of abnormal PAPs before the surgery. Do not confuse having a PAP smear with the annual GYN physical exam. Most doctors will collect a PAP smear every 3-5 years after hysterectomy. How do I know when I am in menopause? Menopause does not just happen overnight, your hormones and cycles change over a period of time. But the true definition of menopause is no periods for one year; for those of you without a uterus; symptoms and hormone testing can make the diagnosis. If you are still having periods but suffering the symptoms you may be experiencing peri-menopause. I can offer you options based on your own symptoms and individual concerns or risks.
