Gujarat: The Hub of Medical Tourism in Western India Medical tourism is a very popular concept today. India and a few other eastern destinations have emerged as s hotspot for quality medical diagnosis and treatment, at much lower costs compared to the western countries. India in particular has already proved its importance and competence in this sector. The standards of health care facilities available there are outstanding, to say the least. India excels on all counts: it is armed with the best and the latest of the international equipment and facilities, it has the best of the talent – well qualified and innovative doctors, para medics and the necessary infrastructure to provide almost each and every solution for any health problem. What is striking is the human touch involved with the medical treatment and care. It is heartening to know that doctors in India have performed successful surgeries on patients from the UK, USA and many of the European countries who were often turned away by the hospitals at home, for one reason or the other. The range of these surgeries or procedures include Cardiac to Gynaecology, Orthopedics to Joint replacement, Ophthalmology to Dentistry, Plastic to Gastric surgery (for treatment of Obesity) and so on. The research being conducted there for newer and better, innovative ways of treatments have been instrumental in enabling patients to recover fast, minimising their stay at hospitals to just a couple of days and getting back to normal life sooner than expected, in a highly comfortable condition. The support system is very warm and courteous, helping patients from preparing their travel plans to making their stay completely comfortable, taking total care in case a patient would be alone. The state-of-art facilities are now available not only in the Indian metros but also in most of other

state capitals and major cities in states. In Gujarat, apart from Ahmedabad, the virtual capital of the state that already boasts of quite a few well known hospitals for their overall excellence, the best of the facilities are now available in other major cities like Vadodara, Surat, Anand, Nadiad, Rajkot and so on. I’m glad that Gujarat Samachar and Asian Voice are bringing out the third annual special issue on the subject, a testimony to the fact that both the advertisers, i.e., doctors and hospitals, and patients have immensely benefited from it. We are thankful to Dr. Bharat Mody, Consultant Joint Replacement Surgeon (Centre for Knee & HIP Surgery, Vadodara) for providing useful insight on medical tourism in India. India: A silent healthcare revolution in the making “It was the best of times, It was the worst of times!”. Thus wrote Charles Dickens in his famous novel, ‘A Tale of Two Cities’, which is written in the backdrop of the French Revolution! It would seem that this statement would aptly describe the revolutionary changes occurring in the healthcare sector of India. For the first fifty years of its existence after independence, the healthcare infrastructure of India was primitive except for the tier one metropolitan cities like Mumbai and Delhi. Skilled doctors and technologically advanced hospitals were only available in these large cities, and the population of a country, as large as India, had to suffer the hardship of traveling to costly cities like Mumbai to access treatment for almost all health problems of a complex nature. Not only did this induce tremendous hardship on the population, but also a very high financial cost due to the extremely high cost of living in these metropolitan cities. It was good news only for the doctors and the hospitals of these cities, who earned healthy incomes (to say the least) due to their monopolistic position. It was the best of times for them, although the worst of times for patients! The driving factors for this phenomenon of unreasonable cost of treatment in metropolitan cities was precipitated by astronomical real estate prices which had a cascading effect on the pricing of every aspect of human activity. Another reason was that highly educated and skilled doctors were not inclined to live their whole lives in smaller cities due to lack of good quality educational and recreational opportunities. However, in the last ten years a silent revolution has swept the Indian healthcare landscape. A long list of small and medium sized cities have rapidly evolved as the new meccas of health care. Vadodara, Nasik, Coimbatore, Nagpur, Kanpur and such other relatively low profile cities have blossomed into highly respectable destinations of healthcare offering world class expertise. It is an indication of overall progress that India has made that these small cities have evolved into very attractive urban centers which offer a desirable mix of reasonably priced real estate with good quality education and the latest recreational facilities. This has attracted highly skilled doctors to make their careers and lives in these smaller urban centers. The extremely attractive combination of high quality world class medical care at very reasonable prices delivered without the hassles of handling movement and accommodation in a difficult city like Mumbai or Delhi has virtually reversed the flow of patients from metropolitan cities to these smaller cities. A classic example of this phenomenon is the story of the city of Vadodara and an Orthopaedic Surgeon named Dr Bharat S. Mody. AFFORDABLE SPINE SURGERY at the hands of

NORTH AMERICA TRAINED SPINE SURGEON A young 21 year old adult, presented with persistent leg pain since last one and a half years, with mild associated backache. He had visited many surgeons in an attempt to get a cure, but was not relieved. He was investigated and diagnosed as having a disc prolapse in his lumbar spine, causing sciatica down his rt. leg. Dr. Subir Jhaveri performed a microendoscopic discectomy (fragmentectomy) using a minimal invasive approach, which relieved his symptoms right away. He was allowed out of bed the same evening and went home the next day, with a tiny incision of less than 2 cm. on his back. currently practising exclusively spine surgery, at Ahmedabad, India. He is affiliated with most of the corporate hospitals of Ahmedabad, where he undertakes major spine surgeries. Besides undertaking minimal invasive discectomies and degenerative spinal surgeries, he also performs advanced spinal reconstructions on a routine basis. He has special interest in the correction of spinal deformity (Sclliosis) as well as tumour surgery. Medical Tourism: Myth or Reality? India has emerged as one of the very few countries that have beaten the economic meltdown and strengthened its position as a robust economy in the world. Amongst many of the growth sectors that have propelled the economy, health care remains a major contributor. Medical tourism has gained momentum in India over the past few years, a trend underpinned by the emergence of new high-quality healthcare service providers and low-cost advantage. Approximately, over 2,00,000 patients arrive in India every year from across the globe for medical treatment. That number is growing at 30 -40% annually. Government of India predicts that India's health-care industry as a whole could grow 13% per annum for the next six years, with the inflow of international patients, which industry analysts say is growing at 30% annually. In 2007, around 272,000 medical tourists are estimated to have visited India for medical treatment and together, they brought in revenue worth US$ 656 million. The Indian medical tourism market is expected to generate over US$ 2.5 billion in revenues by 2012. Market and Research, a consulting firm in USA, reported in a market study that “India offers vast range of medical treatments - from simple dental procedures to the complex cardiac surgeries. Patients can save 60% - 95% of their treatment costs by opting for treatment in India.” Newer and costlier treatments like Bone marrow transplant, Liver transplant and Stem cell therapy are now available in premier Indian hospitals at a fraction of the costs in the West. Interestingly, the end results of these procedures and therapies performed by Indian doctors match the best in the world! JCI (Joint Commission International) of USA, a division of JACHO - an apex agency which accredits the hospitals in USA, has given its accreditation after tough inspection procedures, to a number of hospitals in India. JCI seal ensures comparable US standards for the quality of care and patient safety of these Indian hospitals, which includes six Apollo Hospitals in India. India now enjoys a considerable superiority over South Africa, Mexico, Singapore and Thailand as preferred medical destination in medical tourism market with similar technology, quality care, renowned hospital brands and excellent outcomes – all at a price these countries can not match! The future of Medical Tourism in India looks very bright as the hospitals in western world continue to remain overburdened and there is no check on spiraling cost of health care and health insurance premium in USA and elsewhere. TIME magazine has recently commented “US Hospitals gobble up a third of all medical expenditures. At current rates, the U.S. will be spending $1 of every $5 of its GDP on healthcare by 2015, yet more than 1 in 4 workers will be uninsured…. Therefore the solution lies in a health insurance plan for the uninsured that will combine primary and emergency care in the U.S. with surgery abroad..” India stands to benefit a lot if US companies, desperate to cut costs for health care of their work force, get insurance and government backing to outsource some of their healthcare needs to countries such as India. In countries like Kenya the health care costs are beyond the reach of a common man, the end results are unreliable. Insurance companies in 18

Africa send their patients to Indian hospitals whereas earlier they used to send them to South Africa and Europe. The results have remained the same, while the costs to insurance companies have gone down significantly! Recently King Henry Mulago of Uganda chose to fly to Apollo Hospitals in Ahmedabad for the treatment of cancer! The hospitals also offer assistance with visa application, airport transfer and other formalities. Medical Tourism web portals and agents have mushroomed of late and are attracting clients globally. They offer value addition to the medical treatment including the tourism in India. These companies have tie-ups with doctors, hospitals, travel agents and tourism companies in India and they offer comprehensive service to the international patients and charge appropriate fees to the patients. Hearing loss is a physical disability that is not visible, and in many cases, also not curable. There are three major types of hearing loss: Conductive; Sensor neural and Neural hearing loss. In Conductive hearing loss, the problem is caused in the outer or middle ear that prevents it from conducting sound properly. They are usually mild or moderate in nature. In some cases, conductive hearing losses can be temporary. Medication or surgery can help, depending on the specific cause or the problem. It can be helped with hearing aids. In Sensor Neural hearing loss, a problem in the cochlea (inner ear) is the cause. Sensor neural hearing loss results from missing or damaged sensory cells (hair cells) in the cochlea and is usually permanent. It is also known as “nerve deafness”. It can be mild, moderate, severe or profound. It cannot be cured by surgical procedures, while medication may be helpful in some cases. Mild to severe sensor neural hearing loss can usually be helped with hearing aids. Severe or profound hearing loss can usually be helped with cochlear implants. Neural hearing loss is a profound and permanent. Laser Technology for Prostate surgery Prostate is a walnut sized gland in a male body, just below bladder through which urethra passes. Enlargement of the prostate is a part of the aging process due to which patient feels difficulty in passing urine, thin stream of urine, obstruction in voiding, burning pain while passing urine and sometimes retention of urine. The patient has to undergo certain investigation including uroflometry and total S.P.S.A level to determine the type of treatment needed. Surgical treatment for enlarged prostate is needed when there is a urinary tract infection, significant residual urine after voiding, passing of blood in urine, increase of creatinine and blood urea levels as also when it is associated with bladder stone. For management of enlarged prostate, normally, conventional endoscopic resection of prostate (TUR-P, Trans Uretheral Resection of Prostate) is carried out by urologists under anaesthesia. An endoscope is passed through urethra and with the help of resectoscope, chips of prostate tissues are resected and all chips are removed. Now latest evolution for treatment of enlarged prostate is the introduction of “Laser Technology”. Laser treatment is one of the most advanced, minimally invasive surgical procedure for symptomatic Prostate gland enlargement. Success rate with laser treatment of prostate is very high when compared to conventional endoscopic method. This treatment for prostate is very safe, easy and a highly effective minimal invasive therapy. Especially laser therapy is beneficial for larger prostates (more than 5060gms) as the complication rate is much lower when compared to conventional endoscopic resection of prostate (TUR-P). In TUR-P, time required in surgery is longer, complication rate is much higher as patients may develop TUR-P syndrome. In such a situation, patients develop low sodium level in blood (Hyponatremia). There is insufficient resection of prostate with conventional endoscopic method in larger prostate gland, while with laser treatment, one can achieve complete removal of prostate adenoma. Here in laser therapy procedure, time is shorter and fluid used for operation is normal saline. So there is no incidence of TUR-P syndrome with laser therapy, blood loss is significantly less even in a larger gland (more than 100 grams), almost eliminating blood transfusion requirement, while secondary hemorrhage & re-operation rate is very low. Post surgery catheterization time & hospital stay in laser technology is only one day. For normal, healthy patients, it is an outpatient 26

procedure. Now with this newer laser technology, complete removal of even a large prostate is possible. Compared to conventional endoscopic method with laser treatment, there is minimal thermal injury to surrounding tissue. Laser treatment is equally safe and effective alternative for the minimal invasive management of enlarged prostate, especially in cardiac patients receiving anticoagulant therapy. Patients quickly return to normal activity after laser therapy”. Reported incidence of impotence is less than one percent. There are three types of laser machines available in the market for treatment of prostate. Thulium laser machine, holmium laser machine and greenlight KTP (Potassium Titanyl Phosphate) laser. Again these machines are of two types - High power (100-120 watt) and low power (30-80 watts) machines. Low power machines are not very much effective in removal of larger prostate gland and operative time is much longer while high power Thulium, Holmium or Greenlight HPS-KTP laser are faster and more effective for larger glands. High power laser machines are safer in high risk and cardiac patients. If one has to select by which laser machine he should get prostate surgery done then high power (120 watt) Thulium, Holmium or KTP laser is recommended. It is very safe and effective for prostate. High power holmium (lOOwatt) laser is also an alternative therapy for prostate than low power holmium laser (30-80watt). Our Mission – Save Vision Five decades ago, it all started as just a recreation club for blind people in Ahmedabad. And today, it has grown out into a giant voluntary organisation for not only the blind, but also for people with various types of disabilities. Blind People’s Association (India), Ahmedabad is a multi faceted organisation that works for the overall development of disabled people, helping them with early intervention, education, training, employment, community based rehabilitation. BPA has set it’s mission to help people with every type of disability, while their prime focus remains on saving and regaining vision for a vast majority of the under privileged, poor people of the country who have very little or no access to the health care facilities. Cataract has been identified as one of the major reasons of preventable blindness. In fact, BPA has pledged that the organization will help each and every citizen who needs support, with cataract surgeries and implanting Intra-ocular lens in their eyes to help them save, regain their vision. The Association has set up a well-equipped facility, known as Navalbhai and Hiraba Eye Hospital at Bareja village, at a distance of just 25 kms. from Ahmedabad and well connected to the city with it’s location on the National Highway No. 8. The hospital provides free surgical services to all the needy people. More than 11,000 eye surgeries have already been performed here. The Association has also received over whelming response for it’s services from the needy masses and donors, patrons as well. Looking at the need to provide cataract surgeries to the deserving people, the Hospital has set for itself a goal of performing 10,000 cataract surgeries in the year 2009 alone. BPA now performs surgeries with the latest practices, i.e. “Small Incision Cataract Surgeries (SICS)” and also ensures implant of Intra ocular lens (IOL) with all the surgeries. With support from various quarters, unbelievable it may sound, but a complete Cataract Surgery just costs Rs. 1300 (Approximately £13). This includes all pathology, surgery, medicine, hospitalization for 2 nights, meals for the patients and also necessary follow-up treatment. A very small cost by all standards and yet a very big service to the deserving masses, who once again can see the beautiful world and become self reliant. BPA welcomes help and support from all in their zest to prevent cataract induced blindness. It would be a donation for a worthy cause.
