A HANDY RESOURCE AT YOUR FINGERTIPS Sarah May, Deputy Chief Executive of the IBMS, explains why diagnostic industry partners play such an important role and how vital it is to look beyond the walls of the laboratory.

I

am writing this while still basking in the residual glow of Congress 2019 – possibly our most successful recent Congress. There are several very good reasons why Congress 2019 was such a success, but one that stands out in particular is the role that was played by our diagnostics industry partners. Without our industry partners supporting us with their amazing input to the exhibition, there would simply not be a Congress as we know it. However, industry did more than simply provide our exhibition content, for the first time they had a significant input to the lecture programme and workshops. We have so much to learn from our industry partners and they have so much to offer. It is very easy to become blinkered to

life beyond the laboratory, and the diagnostic companies that provide us with our equipment and reagents have knowledge and experience that is usually from a very different perspective from our own. Engaging with our representatives and contacts in the industry sector can broaden our understanding of the multiple and complex factors that shape the delivery of healthcare and that enable us to have a truly holistic view of our sector.

RISE TO THE CHALLENGE IN THE YEAR AHEAD Mark Reed, Chairman of the Company Members Committee and General Manager of Pro-Lab Diagnostics, says it is a time of change and challenges, but the future will be bright.

N

ow in its third year, the IBMS guide is certainly proving useful to all members be they laboratory or commercially based. I have said on behalf of the Company Members many times how valuable our relationship is with IBMS and that has not changed. The thoughts shared by Sarah here clearly reflect the value with which all parties hold the strong relationship that we have, and Congress is without doubt the key event in all of our calendars every two years. What does continue to change, of course, is the world of biomedical science and the use, application, and development of vitro diagnostics kits and reagents. Consolidation, standardisation, mergers, new

technologies, automation, genomics, a need for faster results, clearer patient care pathways continues to be a challenge for all concerned. When it will all settle down, no one really knows, but we must continue to rise to the challenge. This drive for improvement is punishing at times, and it does require high levels of investment in research and development, marketing, regulatory compliance, and market acceptance once the

products are ready for market. It is hoped that all involved parties continue to recognise this, and that we don’t become a market completely dictated to by cost. The future of biomedical science will be bright, and will no doubt be nothing like the traditional market of science some of us will remember, but it will be there, and so will we. My final point here is one for recruitment, if I may. Your Company Members Committee (see pages 8-9 for more information) is a small group of individuals and we could do with some help. Please let me know if you have some time to spare – it is rewarding. Be warned though, it can also be hectic, especially on the run up to Congress. Enjoy.

INTRODUCING THE COMPANY MEMBERS Who are the Company Members and what is the aim of the scheme? We give an outline of their role and responsibilities.

uring 1992, the Institute of Biomedical Science launched the Company Membership scheme and the Company Members Liaison Group, which comprised Company members and Institute Officers.

The latter held its first meeting in April 1992, which provided an excellent forum to both discuss issues at the Liverpool Triennial and work with exhibitors to transform the event. Fast-forward to the present and the scheme connects a members’ network of over 20,000 biomedical staff

29 networks NHS England and NHS Improvement are working towards the creation of 29 hub and spoke networks, as analysis showed there is unwarranted variation in how NHS pathology services are delivered.

In 2018 specialist trusts were asked to work with designated local networks and consider the wider opportunity for working nationally. We assessed criteria such as walking time, hands-on time and existing infrastructure. We chose a bench along the back wall, adjacent to all the microtomy stations. This kept digital pathology centralised.

Slide preparation and scanning

from specimen reception through to reporting. Leeds used LEAN principles to assess the workow and evaluate how digital pathology would be incorporated. It is important that digital pathology is not considered as an additional step when planning your digital workow.

Scanner location Where to put the scanners is crucial; it reinforces that the digitisation of the glass slides is the ďŹ nal step in the laboratory process. At Leeds, we conducted a formal options appraisal of 10 locations.

Biomedical scientists and support workers know what constitutes good slide preparation, and the same principles apply when preparing slides for scanning. Tissue sections should be thin and placed in the centre of the slide. There should be no broken or overhanging coverslips, and no excess mountant around the edges of the slide. These steps maintain the lifespan of the scanners and avoid unnecessary delays. It is also important that there is no dirt or excess wax on the slides, which will cause the images to be out of focus. All slides at Leeds are checked and cleaned if necessary during the quality control procedure before being sent to the scanner.

POINT-OF-CARE TESTING Point-of-care testing has been one of the big stories of diagnostics recently. Here, we look at the latest developments.

A

number of research and market analysts have published reports on the projected growth of the point-of-care testing (POCT) sector over the next five years. Grand View Research said the global POCT market was valued at $18.09bn in 2018 and expects it to rise to $22.8bn by 2025. Its report states: “Continuous developments in research and development activities to miniaturise molecular and professional

diagnostic tests is anticipated to drive the demand for the devices. “In addition, improvements in lab automation techniques coupled with introduction of cost-effective and high-quality medical solutions is set to propel the growth.” Other analysts are even more optimistic about growth. For example, Global Market Insights expects the sector value to hit $36.6bn by 2025. It predicts a that the most growth will be in microfluids technology (9.2%), followed by

the tumour and cancer segment (7.6%) and then prescription-based testing sector (7.5%). It states: “Growing awareness about early disease diagnosis and prevention has stimulated the demand of POCT molecular diagnostics for detection of cancer and infectious disease at early stages. It has also benefited genetic testing and detection of sexually transmitted diseases. POCT has become an integral part of disease management and therapy. Applications include drug regimen selection, patient stratification, therapeutic monitoring, and detection of predisposition to disease.” However, the report does go on to stress that there are some barriers to growth: “High cost of product development will be a major market hindering factor. Integration of advanced technology into these devices to reduce the rate of errors have increased the cost of production, which has in-turn led to an increase in the price of these devices, thereby restricting the overall market growth.”

UK developments The growth of POCT in the UK was evident last year when for the first time the IBMS ran a dedicated POCT programme at Congress. Scientists crowded into a packed hall to hear a number of speakers cover a variety of topics from how

POCT has become an integral part of disease management and therapy POCT is introduced and controlled in primary care, to accreditation of POCT services, to the importance of software developments. Dr Sarah Glover is a clinical lead for POCT and was the chair for the Congress POCT programme. She said: “We’ve had individual session that have been well attended and POCT is a discipline which is growing and growing and that’s only going to continue going forwards.”

One of the speakers was Ben Courtney, Accreditation Manager at UKAS, who said “the UKAS approach is adaptable and collaborative with POCT”. He went on to explain: “We want to understand what the problems are and devise an accreditation programme that suits biomedical scientists’ needs. Those thinking about seeking accreditation should conduct a gap assessment and then address those gaps – we

There are so many different ways that POCT can be delivered and will work.” He added that those who are unsuccessful in seeking accreditation should not worry about their lab accreditation – “think of it as an extension to scope,” he stressed “it has no impact on the accreditation of your lab”.

Resources With such fluidity and uncertainty around POCT, in January this year the IBMS launched a Certificate of

Expert Practice in Point of Care Testing to support members in developing their training skills and basic knowledge. Lee Peters, Section Manager in Swansea Bay Health Board, who is currently completing a doctorate in POCT education for laboratory staff, outlined the qualification and explained the reasons behind it. “There have been calls for a POCT qualification for some time, it’s always been bubbling below the surface,” he said. “But because of the scope and the increase in use and demand we thought that a POCT qualification was

Expect the best

High expectations generally lead to higher outcomes. Anyone going into a negotiation should do so with the aim and assumption that they will be able to leave the table with the result that they wanted.

Never set a range

When negotiating on price, never give the seller the price range you were hoping to secure. For example, if you say your expenditure has to be somewhere between £8,000 and £20,000, the seller will just aim for the higher figure.

Make the first offer

Research from Harvard Business School demonstrates that, on average, the person who makes the first offer in negotiations is person who gets nearest to their intended outcome. The theory behind this is that the first offer provides an anchor for negotiations. So, if you put in the first offer (a low anchor)

It is important to stick to facts and not let emotional arguments steer negotiations in an unintended direction. It is easy to fall victim to these when under pressure, so stay calm and remain objective.

Don’t just focus on cost

Cost is a vital (arguably the most important) aspect of negotiations to procure goods or services. But it’s not the only thing to take into consideration. For example, if you are buying an expensive piece of equipment, it could save money in the long run to pay more up front, if a better after-sale services is provided, which ensures you get the best use and value from the equipment.
