Our Clinical Approach Our specialist multidisciplinary teams provide transition-focused healthcare, including specialist assessment, treatment, rehabilitation, and education as well as bespoke step-down services. Throughout all our services we structure our care around Care Pathways specific to an individual service user’s assessed needs. These pathways allow us to outline anticipated care, set appropriate timeframes, and help our service users move progressively through a clinical experience to positive outcomes.

SERVICE:

HEATHERWOOD COURT

A low secure forensic unit for both men and women, including locked rehabilitation and controlled egress.

In addition to nursing and support staff, our multidisciplinary team of over 150 people also includes:

Purpose-built, Heatherwood Court provides an intermediate step for individuals on a mental health treatment pathway coming from a medium or low secure environment, who need further physical and relational security as well as therapeutic support.

Located in countryside close to Cardiff, Heatherwood Court offers up to 47 beds across four single-gender units.

At Heatherwood our focus is on rehabilitation and recovery. By providing our patients with relationship and life skills, alongside resilience and coping strategies, we promote the emotional stability which can provide the sound foundation needed to make a successful move back into the community.

Heatherwood Court’s Recovery College is an initiative that has been co-produced and co-managed by our service users and our staff, with everyone working together as equal partners to decide what the College should aim to do and how we should do this.

SERVICE:

ST PETER’S HOSPITAL

St Peter’s Hospital in Newport, South Wales offers 51 beds split into single-gender units and provides specialist treatment and nursing care that focuses on delivering positive outcomes for our patients with degenerative neuropsychiatric conditions and ABI. Taking a neurobehavioural approach we are experienced in psychological assessment and behavioural analysis to formulate positive behavioural management plans to support successful transitional rehabilitation of the individuals in our service.

SERVICE:

PINETREE COURT

Pinetree Court is a specialist locked rehabilitation service, providing person-centred care for men and women over the age of 18, with learning disabilities, behaviours that challenge, and/or mental illness. We deliver intensive treatment, therapy, and active rehabilitation within a safe and therapeutic environment. Located close to Cardiff city centre, Pinetree Court offers 29 beds, split into single-gender units. We deliver individually tailored support based around models of care including Positive Behaviour Support, Active Support and Building Better Lives.

Building Better Lives Tailored to create positive measurable outcomes, our BBL programme looks at the whole person and helps individuals to develop the skills they need to move on to a more independent life.

Opened in 2008, Pinetree provides care for adults aged up to 65 with a primary diagnosis of learning disabilities, and who are likely to have been detained under the Mental Health Act. Patients may have complex needs and behaviours that challenge. At Pinetree Court we offer a dedicated 20-week assessment and treatment pathway with a focus

BEECHWOOD COLLEGE

Beechwood College in Sully, South Wales offers inspirational education for young adults with Autistic Spectrum Conditions. Beechwood College’s goal is to ensure their students are supported to access transferable, meaningful learning opportunities in a way that takes account of their unique learning style and the impact their ASC has upon them. A consistent approach to educational support, based upon the principles of TEACCH, PECS and Makaton, is embedded in the College’s ethos.

With the support of their Key Worker residential students are encouraged and supported to personalise their own rooms and participate in a wide range of interests and activities in their leisure time. The team works to create strong supportive relationships whilst also ensuring that students do not feel they lose any of the family and friendship connections they have already made. Where students have agreed therapeutic funding, the College can provide access to a wide range of therapies and medical support that seek to promote skill development and remove barriers to learning. The College offers a broad, balanced, and person-centred curriculum that is tailored to the student’s personal interests with the aim of helping them develop as confident adults, reach their full potential, and integrate happily in the community. Beechwood College is foremost a further education establishment however, for our residential students it is also their home during term time.

We also recognise that our students often have difficulties in expressing themselves and communicating distress or need. Communication can take the form of behaviours that are perceived as challenging. We work as a multidisciplinary team to meet students’ needs by developing a person-centred Positive Behavioural Support (PBS) plan.

We have never met Support Workers who are so kind hearted and thoughtful. The Education and Therapies Departments are also very supportive and are always looking at different ways to help our son progress and gain more independence in his life. Mother of a service user

SERVICE:

OCEAN COMMUNITY SERVICES

Ocean Community Services (OCS) is a specialist residential service with homes around South Wales and Bristol. OCS provides step-down and rehabilitation within a clinical care pathway, based on individual need which promotes recovery and progress. OCS offers support to both men and women with mental health needs, autistic spectrum conditions, learning disabilities and acquired brain injury, including those with a forensic history. Service users come from a variety of backgrounds including their own homes, hospitals, hostels, and other residential services. Some individuals will be subject to the Mental Health Act 2007 relating to discharge, aftercare, and supervision. OCS believes that all its service users are people capable of progress and development and are committed to their rights for privacy, dignity, choice, fulfilment, and independence. OCS believes in working with our service users in equal partnership, not making them passive.

This is my longest ever placement, I’ve been at Westminster Court now for nearly two and half years. With the support and encouragement of the staff here I feel I’m finally making progress. Service user

MAKING A REFERRAL We aim to provide free assessment within 48-hours of a referral being received in writing. The comprehensive assessment will cover nursing, clinical psychology, and psychiatry. Following the initial assessment and discussion with the Multidisciplinary Referral Panel, a decision will be communicated immediately to the referring professional. Detailed assessment reports, including treatment recommendations and care pathways for each individual referral are then provided to Commissioners.

In exceptional circumstances where there is a very urgent requirement, assessments can be undertaken more quickly.

All new referrals are discussed weekly by the Multidisciplinary Referral Panel, focusing on identifying need, risk assessment and patient compatibility.
