Family Presence (Open Visiting Policy)

Erie Shores HealthCare (ESHC) is committed to working with patients and families to advance compassionate, high quality care. We believe it is important for patients to experience the support of family and friends, and to be treated as individuals with unique needs. Families and other partners in care are welcome 24 hours a day according to patient preference. The number of people welcome at the bedside at any one time will be determined in collaboration with the patient and family. In situations where there are shared rooms, the rights and needs of the other patient(s) will be taken into consideration. Family who are supporting a patient between the hours of 8 p.m. and 6 a.m. are asked to remain in the patient’s room advising staff when leaving the unit. ESHC will invite one family support at the bedside overnight. These individuals are responsible for their own personal needs. Protocols for presence at the bedside change frequently. They will ask you for your personal and insurance information and guide you through the registration process. Once your registration is complete, you will be directed to your room or the appropriate department.

Patient Wristband

Your health and safety are of paramount importance to us. You will receive a patient wristband listing your details at admission. Doctors and nurses can use this wristband to identify you at any time.

Hygiene and Data Protection

• The wristband is hygienic and non-allergenic. In addition, it is waterproof and tear-resistant.

• We ask you to wear the patient wristband throughout your stay. If it is necessary to replace or remove the bracelet before discharge, you should contact your primary nurse. • A unique Medical Record Number that identifies a patient throughout the hospital system. • A unique barcode. This barcode is your digital identity in our hospital and links to our stateof-the-art hospital information system.

Allergy Wristband (Red)

You will also be given a red wristband to alert your care team if you have severe allergies. If you have an allergy and do not have a red wristband, please speak with a member of your care team.

Your Room

There are different rooms at Erie Shores HealthCare, which include ward, semi-private, and private. All rooms are equipped with a television, direct dial telephone and internet (wifi). We encourage you to bring your device for entertainment but know the hospital internet speeds are slower than standard connections. Upgraded accommodation includes semi-private (2 patients to a room) and private (1 patient per room). Please, check your insurance coverage and present your information at the time of admission. They may cover part or all of the costs. Your responsibility is to know what extended health coverage you have before coming to the hospital. Unlike places like your dentist, the hospital does not have access to your coverage information before your discharge. OHIP does not cover extra costs such as upgraded rooms, or television. Any additional charges are your responsibility.


Spiritual Care

Television

To enjoy your television, operated by the outside agency HealthHub, follow these steps: 1. Go to ConnectMyBed.ca 2. Follow the instructions to select your length of rental time. Have your floor, room, bed, and credit card information handy. Services are refundable and nontransferable to another patient. To request a refund or have your service moved to a different bed, please call 1-866-223-3686 or dial ext. 2000 from your hospital phone. Please note that an outside agency operates television services, and hospital staff cannot help with any related issues. If you have problems with your TV service, please call 1-866-223-3686 or dial ext. 2000 from your hospital phone.

Non-denominational spiritual care at Erie Shores HealthCare provides spiritual support to patients and their families without any specific religious affiliation or bias. It is an inclusive approach that recognizes and respects the diversity of spiritual and religious beliefs among patients and their loved ones. This type of care includes providing emotional and spiritual support, connecting patients with their own personal beliefs and practices, and helping them find meaning and purpose in their illness or injury. Our Spiritual Care team is available for meditation, prayer, or other spiritual practices, as well as connecting patients with chaplains or other spiritual leaders from different faiths. To access our Spiritual Care team, please speak with a member of your care team.

Smoke-free Hospital

Erie Shores HealthCare is a smoke-free facility that includes all indoor and exterior properties. Anyone wishing to smoke must do so on the sidewalk along Fader Ave, outside the main Entrance. For questions on quitting smoking, please call Health Connect Ontario by dialling 8-1-1.

Medical Care Team Approximately 600 staff, including physicians, nurse practitioners, nurses and more, work at Erie Shores HealthCare in various medical fields.

Your entire care team shares the same goal, providing the best possible treatment for all patients. We encourage you to be an active part of your care and welcome any questions about your care you may have. Members of your care team could include: HOSPITALIST

A hospitalist is a licensed physician who manages your care during your stay in our hospital. They cover the gap from your primary care doctor to the hospital or from the emergency department back to primary care.

INTERNAL MEDICINE (INTERNIST)

Internal medicine doctors, or internists, are physicians who specialize in the diagnosis and treatment of diseases threatening internal organs, such as the heart, lungs, digestive and intestinal tract. They provide care for patients with complex medical conditions. You will most often see our Internal Medicine physicians in our ICU and Telemetry Units.

CLINICAL EXTERNS

A relatively new role in healthcare in Ontario, Clinical Externs are students from various healthcare fields who provide support to nurses or other allied health team members. Our externs work alongside our healthcare team, learning on the job while providing essential support to patients and healthcare team. 10 Erie Shores HealthCare Patient and Family Handbook


NURSE PRACTITIONER

Nurse practitioners (NPs) are registered nurses who have additional education and nursing experience, which enables them to:

u Autonomously diagnose and treat illnesses u Order and interpret tests u Prescribe medications u Perform medical procedures

NURSES (RNS & RPNS)

Your primary care providers are our compassionate and dedicated nursing team of Registered Nurses (RNs) and Registered Practical Nurses (RPNs). Our nursing team is responsible for administering medications and treatments, monitoring patients’ vital signs, communicating with physicians and other healthcare professionals, and providing education and emotional support to patients and their families. They also play a role in infection control, patient safety, and documenting patients’ care and progress. Nurses may also be responsible for managing and supervising other healthcare staff.

PERSONAL SUPPORT WORKERS (PSWS)

Personal Support Workers (PSWs) help to support patient care. They assist nurses to lift and turn patients, cleaning and restock patient bedsides and help manage supplies and equipment. PSWs do not provide independent patient care but support patient care in many valuable ways.

DIAGNOSTIC IMAGING TECNOLOGIST

A Diagnostic Imaging Technologist, commonly known as a radiologic technologist, plays a crucial role in your care during your hospital stay. They are specialized professionals responsible for performing various imaging procedures that help doctors diagnose and treat medical conditions. These procedures include X-rays, CT scans, MRIs, ultrasounds, and more.

Other team members you may meet:

Housekeepers A vital piece of your care team, housekeepers maintain a clean environment, limiting the spread of hospital-borne illness. Patient Transporters When it’s time for you to move from one part of the hospital to another, our patient transport team gets you there on time and safely. Lab Technicians These highly trained Medical Laboratory Technologists draw specimens from patients and perform critical tests in our state-of-the-art lab. Pharmacy Tech. Our pharmacy team ensures the right patients receive the proper medications at the right time. Your patient wristband plays a big part in administering your medication. Physiotherapists and Occupational Therapists They help with your mobility, improving or maintaining your mobility and independence for daily activities.

Goals of Care

Goals of care in a hospital refer to the overarching medical objectives and treatment plans for a patient based on their health needs, preferences, and values. They outline the priorities and desired outcomes of medical treatment, and help guide healthcare providers in making care decisions. These conversations occur with every patient who enters the hospital and are frequently revisited during treatment. You can make modifications to your goals of care at any time. You can determine your goals of care by having open and honest discussions with your care team about your health status, medical history, and personal preferences. You can also consider the following when determining your goals of care: • Health status: Patients should consider the impact of their medical condition on their daily life and the likelihood of recovery.

• Treatment options: Patients should understand the available treatments and the associated benefits and risks. • Personal values: Patients should consider their values, beliefs, and goals and how these might affect their healthcare decisions. • Family/support system: Patients should consider the opinions and needs of their loved ones and any impact that healthcare decisions may have on them. It is essential to communicate your goals of care clearly to the care team and have a plan in place in case of an emergency. By being informed and proactive, you can ensure the goals of care align with your healthcare journey.

Food & Beverage Patient Meals

Patient meals are an essential part of your care. Patient meals are prepared fresh daily by our dietary staff in consultation with your care team. If you have any dietary restrictions, please inform your care team.

Cafeteria

Our cafeteria, located on the 1st floor, straight ahead of the elevators, provides self-serve options 24 hours per day. In addition, hot meals are served in the cafeteria daily between 11:30 am and 1:30 pm. The cafeteria is cashless, so a debit card or credit card is required for purchase.

OUTSIDE FOOD & DRINK

Single-serving, pre-packaged food may be brought up for the patient, if allowed - NO external food is permitted for ICU patients. Additionally, no communal food is allowed.


Additional Amenities The Inspirations Gift Shop is operated by the ESHC Hospital Auxiliary and staffed by hospital volunteers. It is located on the Ground Floor in the main hallway. Items available for sale include clothing & gifts as well as everyday essentials. Hours vary depending on the season and volunteer availability.

Lost & Found

Policies During Your Stay Family Presence Policy

Visitors are invited at Erie Shores HealthCare from 8 am until 8 pm. Our family presence guidelines change frequently depending on the current public health outbreak status. Therefore, we recommend checking the website at Erieshoreshealthcare.ca/visitorhours for the most up-to-date information.

Scent Free Policy

Many elements in the environment can trigger or influence health and disease.

If you have lost an item, contact our Patient Experience Specialist, who is available between 8 am and 4 pm. You can reach them by emailing PatientExperience@eshc.org or by calling 519-326-2373 Ext. 4118.

Considering others who may suffer from chemical sensitivities and/or allergies, we request that you refrain from wearing scents such as perfumes, colognes, hairspray or aftershave at Erie Shores HealthCare.

Interpretations / Translation Services

Flowers, parcels, and mail will be accepted at the Main Entrance and delivered to your room by a hospital staff member. Please note: Flowers are prohibited in ICU.

Erie Shores HealthCare (ESHC) offers telephone interpretation services within every department. The service provides immediate access to qualified interpreters in over 170+ languages. Our phone interpretation services are part of ESHC’s commitment to Patient and Family-Centered Care. Phone interpretation encourages the highest standard of language support delivered by specially trained interpreters and allows patients to communicate more effectively with physicians, nurses and other hospital staff. If you or a loved one require interpretation services, please advise a care team member.

Pet Policy

Erie Shores HealthCare recognizes the importance and value a pet contributes to its owner’s well-being and mental health or those who interact with pets. Dogs and cats are the only personal pets permitted for pet visitation. If you are interested in having your pet visit, request a copy of the Patient Pet Information form. You will be required to provide a copy of your pets updated pet vaccination records. This policy does not refer to Service Animals accompanying patients to hospital appointments. Hospital Discharge & Feedback Privacy & Confidentiality

Erie Shores HealthCare is committed to protecting your privacy and confidentiality. We follow all the rules and regulations set out by the Ontario Personal Health Information Protection Act (PHIPA). This means that we will only share your personal or health information with your consent if it is necessary to provide care. We use and disclose your personal health information to: • treat and care for you,

• get payment for your treatment and care (from OHIP, WSIB, your private insurer or others), • plan, administer and manage our internal operations, conduct risk management activities,

• perform quality improvement activities (such as sending patients satisfaction surveys), • teach, conduct research, compile statistics,

• fundraise to improve our healthcare services and programs, comply with legal and regulatory requirements, and fulfill other purposes permitted or required by law. If you have any questions or concerns about your privacy, please speak with a care team member.

After your treatment, a care team member will inform you when they expect your discharge to happen. Patients and families should expect discharges to be fully completed before 11am. Upon discharge, a member of your care team will provide notes detailing any follow-up care. You may also have electronically submitted prescriptions to your pharmacy that you will need to pick up. If home care services are required, hospital staff will make referrals to community partners on your behalf. You are required to arrange your transportation from the hospital. A member of your care team can help you arrange transport if needed. Please note the hospital does not cover transportation costs.

Your feedback is important to us.

The patient feedback survey is a tool we use at Erie Shores HealthCare to gather valuable information from our patients about their experiences with our healthcare services. This survey is designed to help us understand what we are doing well and where we need to improve to provide better care and service to our patients. The survey is given to patients after receiving healthcare services from our facility and can be completed anonymously. The questions in the survey are designed to cover different aspects of the patient’s experience, such as communication with the healthcare team, quality of care, cleanliness of the facility, and overall satisfaction with their visit. Once the surveys are completed, our staff carefully analyzes and reviews the results. We use the feedback to identify areas where we are doing well and areas where we need to improve. Overall, patient feedback is an essential tool that helps us understand how we perform and where we can improve to serve our patients better. At Erie Shores HealthCare, we take patient feedback seriously and use it to improve our healthcare services continuously.

THEN NOW Leamington has had some semblance of a hospital since 1910 when a nurse named Maude Tisdale and her mother opened a small nursing home on Oak Street. It moved to various houses under different owners, settling in a converted hotel named the Hopewell Hospital. According to the newspaper of the day, it was “bursting at the seams” when its owner closed it down in 1943. Tackling the crisis, local service clubs and a new hospital board began a fundraising campaign amid the Second World War. Their work, along with support from the townships of Leamington and Mersea, lead to Leamington District Memorial Hospital opening its doors on April 1st, 1950, completely debt free.

The hospital family and its supporters are proud of the hospital’s past and the gains made over the years. As a result of their hard work, dedication and faith, Leamington District Memorial Hospital transitioned to Erie Shores HealthCare in 2017 – a new name intended to reflect better the geographic area the hospital serves.

How You Can Help

Erie Shores Health Foundation

For 30 years, Erie Shores Health Foundation (ESHF) has believed in the power of generosity to save and change lives. We raise funds to help ensure patients across Windsor-Essex receive the best quality, compassionate care close to home. Because of community support, we can address the needs of the Hospice of Windsor Essex, Erie Shores Campus and Erie Shores HealthCare.

Erie Shores HealthCare

At Erie Shores HealthCare, ESHF funds the purchase of capital and high-priority medical equipment – ensuring the most up-to-date healthcare technology for our patients and physicians.

The Hospice, Erie Shores Campus

By agreement with the Hospice of Windsor Essex, ESHF facilitates the funding of patient care operations at the Hospice, Erie Shores Campus Residence and Welcome Centre – making programs and inpatient care free of cost to patients at their families.

Volunteering at Erie Shores HealthCare

Volunteers provide practical help to patients and visitors, complementing the work of paid staff across Erie Shores HealthCare. They do not carry out clinical work or do work experience. Erie Shores Volunteers are not directly involved in patient care but help to provide extra support. They come from all walks of life and represent the diversity of the communities that we serve. All are dedicated to making our hospitals a little friendlier and more comfortable for everyone.

Who Can Volunteer

We ask you to give us at least three hours of your time a week for at least six months, so please consider whether you can realistically fit this commitment in with your other responsibilities. There is the possibility of volunteering in the evenings or on weekends but please note these are our busiest times and if you are able to volunteer between 9 to 5, Monday to Friday, there will be more opportunities for volunteering.

Our volunteers are confident and enthusiastic people. Our current volunteer base ages range from teenagers to seniors. To volunteer, you need to be able to display some key personal qualities such as

Caregiver Support Services

If you support someone in need and feel anxious and overwhelmed with your caregiving responsibilities, you’re not alone. If you’re a caregiver and are feeling burn-out or overwhelmed, help is available. The Ontario Caregiver Organization is a free support resource. Access their services through the Ontario Caregiver Helpline at 1-833-416-2273 (CARE).

The Ontario Caregiver Helpline is a one-stop resource for information and referrals connecting caregivers to the support you need. This might include respite, a support group in your community or programs for the person you care for. When you access Helpline, a community resource specialist answers your call 24/7 or your online chat request (at ontariocaregiver.ca) Monday-Friday 7am-9pm EST.

Organ & Tissue Donation Register today to help someone in need

Every three days, a person dies waiting for an organ transplant. By registering to become an organ and tissue donor, your death has the power to save or change someone’s life. Facts About Organ and Tissue Donation • 1 organ and tissue donor can save up to 8 lives and enrich as many as 75. • The first and foremost duty of health care professionals is to save a life. Only when a life cannot be saved does organ and tissue donation become an option. • Everyone has the potential to be an organ and/or tissue donor, regardless of age or health. • The majority of religions support organ and tissue donation or respect an individual’s choice. • Organ and tissue donation should not affect funeral arrangements. • Your donation decision is confidential and will not affect your medical care. Your donation decision will only be disclosed at the end of life. • You can change or withdraw your consent to donation at any time.
